  • In this role, I undertake assessment activities in Townsville, Ingham, Chatters Towers, Magnetic Island, Ayr and Home Hill as scheduled under the Commonwealth Home Support Programme (CHSP) level
  • This includes:
  • Prior to the assessment: -Reviewing client’s information as provided during My Aged Care intake either by the client, family member, representative or client’s Doctor
  • Follow-up with relevant person if more information is needed or additional support like translation services or a social worker
  • Check client’s medical history, cognitive and physical impairment affecting their functioning at home
  • Highlighting supports requested as well as those that may be needed due to client’s ongoing health needs and limitations
  • Liaising with client’s support system where a client is limited in communication, language barrier, physically, cognitively or otherwise to ensure their needs are adequately met during the assessment
  • During the assessment:
  • Ensure clients are matched and referred as indicated to either a service provider or for a comprehensive assessment to facilitate an appropriate support pathway by: -Reviewing and updating client’s ongoing medical conditions, medication, falls, hospitalization, family and available carer supports
  • Discuss client’s limitations in relation to supports needed including medication management, medication review (liaise with GP), mobility impairment -needed aids and allied health support
  • Critically discuss and analyze medical conditions that need linking support including dementia -dementia services, diabetes -feet care, vision loss/impairment -link for equipment, incontinence -refer for hygiene aids assessment, mental conditions including grief, PTSD, terminal diagnosis, family related stressors, -refer for psychological support, alcohol and drugs related dependence -discuss referral for psychological support
  • Refer to the Aged Care Assessment Team (ACAT) for a comprehensive assessment, if needs can’t be met at the (CHSP) level
  • Ensure holistic case management is undertaken, focusing on client strengths and abilities to optimize health outcomes, independence and self-reliance: -Discuss informal supports available and suggest task simplification ideas and items to encourage self-reliance and independence
  • Establish re-ablement pathways with goals and realistic timelines to reach agreed milestones for the client, their support system or service providers
  • Send agreed referrals for services that can’t be met informally or through the client’s GP care plan to local service providers
  • After the assessment:
  • Undertake short term case management or care coordination services as required to provide episodic assistance in overcoming barriers
  • Follow-up with clients and service providers one day after the assessment and ensure linking support has been offered for urgent services like medication assistance and personal care
  • Contact clients periodically to follow-up on their health, mobility improvement and ensure goals set for re-ablement are being met
  • Follow-up with service providers to ensure services are being delivered in a timely manner.
