Summary of the Unit

28 Bedded Unit specialising in Acute Medicine for the Elderly and Stroke which offers services ranging from Acute medical intervention to rehabilitation and end of life care. It is under the clinical governance of Stroke Team Consultant and offers full MDT input including physiotherapy, occupational therapy, social work, nutrition and dietetics, speech and language therapy.
Occasional admissions of surgical patients under demanding circumstances mainly patients awaiting for procedures like endoscopic investigations, appendectomy, cholecystectomy etc.

  • Stroke Unit including Care of Elderly, Working as a Nurse Lead for the shift to coordinate comprehensive patient care. Patient allocation ratio (1:5/1:7), overseeing five nurses/ three HCAs during the day, three nurses/two HCAs at night.
  • Providing comprehensive nursing care including full assessment of the patients with Stroke.
  • Care of patients with stroke mainly during window period, post thrombolysis phase and Rehabilitation phase.
  • Perform swallow screening.
  • Attending the ADL's of the patients.
  • Attending ward rounds with medical team.
  • Supervising students and acting as a preceptor by helping students to attain their learning objectives by demonstrating procedures, assisting while giving care to patients, obtaining feedback along with clinical practice coordinator etc.
  • Working as a team leader among the group to facilitate team work.
  • To act as an advocate for the patients and liaise with MDT (PT/OT/SLT/MSW/CNS/ANP) to promote safety and restoration of health.
