The Home Care Registered Nurse provides skilled nursing care to patients in their homes in accordance with physician orders, individualized care plans, and applicable regulatory and organisational requirements. The role includes comprehensive nursing assessments, wound management, catheterisation, insulin management, monitoring of falls risk and nutrition, oversight of restrictive practices, and coordination of care with physicians, allied health professionals, patients, and families. The RN demonstrates professionalism, adaptability, and diplomacy when managing complex clinical and interpersonal situations.

• Conduct initial, ongoing, and reassessments of patients in the home setting
• Assess physical, functional, psychosocial, cognitive, and environmental factors affecting care
• Monitor and document falls risk, nutritional status, skin integrity, mobility, and safety concerns
• Identify changes in patient condition and escalate concerns appropriately

• Perform wound assessments and monitor healing progress
• Provide wound care and dressing changes in accordance with care plans and physician orders
• Educate patients and caregivers on wound care, pressure injury prevention, and skin integrity maintenance

• Perform catheterisation and provide ongoing urinary catheter care as prescribed
• Provide insulin management, including administration, blood glucose monitoring, and patient education
• Deliver skilled nursing interventions within home care scope, organisational policy, and professional standards

• Assess and monitor falls risk and implement appropriate risk-reduction strategies
• Monitor nutritional intake, hydration status, and weight changes, and report concerns to the care team
• Identify, document, and monitor restrictive practices in accordance with legislation, organisational policy, and care plans
• Advocate for least-restrictive, person-centred care and escalate concerns as required

• Liaise with treating physicians, allied health professionals, case managers, and support staff
• Communicate assessment findings, risks, and care needs in a timely and professional manner

• Graduate of an accredited nursing program

• Competence in wound management, catheterisation, insulin management, and risk assessment
• Strong communication and coordination skills

• Strong assessment and clinical reasoning skills

• Deliver direct and comprehensive nursing care and individualised clinical case management to village residents.
• Manage medication wound care and other clinical services to residents.
• Lead, mentor and train junior RNs running micro sessions, group activities and competency assessments.
• Work with the Village Care Manager to identify gaps in training.
• Develop program of clinical audits in conjunction with the National and Village Care Managers and implement them according to the program.
• In conjunction with the Village care Manager and Palliative Care Consultant, ensure all aspects of palliation are delivered to the highest standard.
• Support the Dementia Manager and all care staff to deliver best practice dementia care.
• Provide clinical leadership for Personal Care Assistants on shifts as required including attending to incidents or issues requiring clinical escalation, ensuring compliance with incident reporting processes and escalating issues to the appropriate person in a timely manner.
