  • Understanding the person’s current health status, lifestyle and personal goals through intake and/or bedside visit at the hospital and/or over the phone (Canberra Hospital, North Canberra Hospital and University of Canberra Hospital)
  • Setting a goal to identify what the person wants to achieve after the 6 weeks with PHSP
  • Outlining the services and supports needed to reach the client’s goals.
  • Putting the plan into action such as referrals to liaised providers for personal care, domestic assistance, transport and meal preparation as needed
  • Regularly checking in to see if the plan is working and making adjustments as needed
  • Making sure to meet KPI by keeping track of the plan and by creating tasks on the client’s profile in Dynamics
  • Making sure to contact each client within 48 hours of receipt of the referral
  • Writing a Care Plan document after every intake including the goals and objectives
  • Doing the necessary referrals to another Community Assistance and Temporary Support (CATS) provider for bridging support or care coordination while the client transitions to either my aged care (MAC), NDIS or to recovery
