Summary

I completed my bachelor’s degree in social science, Majoring in Community Welfare and Human Services in 2017. I'm a confident and reliable worker who is highly motivated about making a positive and effective difference within people's lives. I have over eight years of experience working in customer service and seven years working as a level 2 coordinator of supports, recently becoming a specialist level 3 coordinator of supports in March of 2022, which has given me skills in which to resolve problematic issues/situations as well as developing an open and understanding outlook on complex issues. I'm a fast learner and enjoy all opportunities in which will challenge myself and continue to learn along the way, working within a team so that I will continue to learn from people with experience and advanced knowledge within the field as well as working individually are both areas in which I will happily thrive. I'm task orientated and organized due to my passionate drive to work with and support people within my community. My main goal within my career is to ensure those I’m working with are thriving to achieve the most they can and more.

  • Communication between all service providers and participants/families is vital within my role. Constant communication is imperative to ensure all information is smoothly transferred and the correct services are provided.
  • Reiterating the above, one of the main principles instilled within my role as support coordinator is to ensure “Choice and Control” is implemented for all participants. It is my job to advise participants and families of all options available but to allow independence within their overall decisions. The tools I used regarding this is active listening and open discussion between myself and all involved. This is important as it allowed for open discussion and an overall positive environment for the participants to make informed decisions.
  • While working within Trilogy, as well as continuing to work closely within the realm of the NDIS, I also work with participants who are under the care of the Minister (DCJ), Out of Home Care (OoHC) and Voluntary Out of Home Care (VOOHC) as well as clients utilizing aftercare plans through DCJ.
  • I have also worked closely alongside other human service sectors through my role as an NDIS CoS some are NCAT, employment, medical/hospital services, housing and education. Although there are clear lines between these areas within a person’s life and NDIS plan, they are all intertwined in a client’s overall needs/Goals so ensuring communication, meetings and updated information is shared and available ensures my clients have the ability and confidence in accessing services in all areas.

  • Through both roles within this company, I’ve gained experience in linking people with disabilities to services provided under the NDIS as well as those not funded under the NDIS. This experience gave me the skills regarding making and maintaining positive relationships between participants/families of participants and services.

  • While employed within ConnectAbility Australia (Erina hub) as a disability support work, I was able to become an integral part of my clients’ lives, directly focusing on their NDIS goals as well as their own life goals. This role was an integral part within my NDIS journey, to see firsthand the impacts both positive and negative can have in regards to the services and supports in place/not available and how this impacts the clients progress/overall health and well-being. This assisted in my transfer into a support Coordination role, as I was able to utilize the invaluable experiences as a support worker, to assist with behind-the-scenes work; the report writing, the needs for assessments, data collection and continued communication between all stakeholders, family and the individuals themselves.

  • Disability support experience, working with vulnerable people to achieve/reach their chosen goals this position has enabled me to support participants with high to low level needs. This experience has also taught me skills in working with people with physical and intellectual disabilities during intense and confronting situations.
  • Time management is key within this role as the participants you work with are relying on gaining quality support within their designated time allocated. These participants are given a certain amount of funding which allows them to gain support services. Ensuring time isn’t wasted within these supports ensures they’re receiving the most out of the services.

  • During my placement I was given the opportunity to gain experience within the OoHC team at FACS Gosford. During this placement I was able to observe and draft Learning, School, and Health care plans. I observed and completed supervised home visits, in which I took notes that I later compiled into online and paper files which could be accessed by all participants. I observed a mediation process and contact visits which enabled me to observe high intensity conflict situations as well as learning positive/innovative resolutions.
