1.Delivering clinical care to clients

2. Care planning and coordination for clients

3. Advocating for clients living in their home

4. Regular monitoring of client's health and documentation in Alaya Care

5. Regular reporting to area manager and quality team regarding clients nursing needs

6. Conduct Nursing assessments, care planning and follow ups of HCP clients

7. Monitor, Support and Guide EN and CSEs relating to Nursing care for clients when needed

8.Participate in networking programs to promote Baptist Care in community setting

9. Work in conjunction with Care Facilitators to provide the best clinical care needs to clients

· Work by Organizational policies and procedures and standards of practice

· Provide complex clinical care including but not limited to wound dressing; administration of medications; nursing assessment and referral.

· Maintain accurate and appropriate record keeping and documentation

· Liaise with Medical Practitioners, hospitals, Pharmacists and other health professionals about complex clients

· Administer Oral, IM, SC, S8 medications for clients requiring medication administration

· Deliver, promote and provide holistic care that focuses on the requirements within the rights of the clients
