Clinical Role

• Provide high level of individual therapy service in all assessments, treatments, and communication.
• Ensure high quality, person and family centred care is delivered to all clients in accordance with best practice
• Liaise directly with associated medical professionals, insurance companies, relatives, treating team and multidisciplinary team.
• Navigate NDIS efficiently and in best interests of client and business.
• Refer to other disciplines and programs, as appropriate, to ensure optimal care and service delivery.
• Organise follow-up appointments, complete reporting and written communication and any case management, administration and liaison that is required.
• Remain up to date with evidence-based practice; maintain excellent level of clinical expertise and skill.
• Maintain continuing professional development requirements for AHPRA registration and self development.

Team Management

• Support growth, wellbeing, service development and quality of Community Physiotherapy Team.
• Coordinate and prioritise Community Physiotherapy Team caseload and allocation of new clients and internal MDT referrals, alongside Intake Team.
• Contribute to staff meetings and report on team KPIs, profitability, productivity, team wellbeing, quality of service and team learning needs.
• Responsible for leave management and overseeing supervision, peer review and performance review process for Community Physiotherapy Team, alongside Physiotherapy Supervisors.
• Oversee on-boarding and off-boarding of staff, alongside Physiotherapy Supervisors.
• Attendance at monthly Leadership Team meeting.

• Work alongside Management Team to develop and implement service development strategies to support growth, wellbeing, skill set and innovation of team.
• This includes development of ARC processes, funding opportunities, partnerships and relationship building and areas of identified skill need of team.
• Participation in Special Interest Groups to ensure that outcomes are delivered in timely manner and supportive of atmosphere of ongoing innovation and quality assurance

Team advocacy

• Development of strong relationships with Community Physiotherapy Team, AHM, People and Culture Manager, other Team Leaders, and wider multidisciplinary team to best represent clinicians and needs of team at all leadership and management events.

Marketing and PR

• Development of strong networks with referrers, suppliers and other allied health professionals and specialists

• Conducted clinical assessment and evaluation to determine and manage patient needs
• Used clinical outcome measures to monitor patient progress
• Attended weekly Stroke and Extended Care Programme (ECP) meetings with wider MDT
• Set client focused goals with patient and their families
• Adapted therapy sessions and exercises according to patient progress and limitations.
• Worked holistically with other professionals to achieve client focused goals
• Tracked and evaluated patient progress with respect to treatment goals.
• Participated in weekly in-service training and journal clubs
• Supervised junior members of staff to aid with their learning and development
• Reported complex issues to senior management and engaged in remediation.
• Researched and stayed abreast of current treatment modalities by attending professional development opportunities..
