I have been employed as a Clinical Nurse within Youth Community Assessment Treatment Team (YCATT) within NMHS since July 2025. As this is my first role working within the community as a clinical nurse, it has been a great opportunity to expand my knowledge and education regarding the delivery of care for young people with complex mental health challenges. My role during the young person's journey within YCATT is to conduct comprehensive mental health assessments, gathering detailed information regarding the young person's mental health, psychosocial background, medical history, and presenting concerns. My role involves conducting thorough risk assessments and safety planning with the young person to reduce the necessity for inpatient hospital admissions, if avoidable. This role requires a balance of direct patient care, ongoing monitoring of mental state, and collaboration with the multi-disciplinary team and their legal guardian/NOK to ensure the best outcomes for the young person's care and treatment, identifying the appropriate interventions alongside their short- and long-term goals. I have managed young people with complex mental health challenges, which include diagnoses of MDD, EUPD, psychosis, schizophrenia, substance use, eating disorders, OCD, ASD, ADHD, depression, anxiety, cPTSD, and PTSD.

• Accept responsibility for maintaining and strengthening one's own clinical practice, competency, knowledge, and expertise in the area of clinical specialty.
• Appropriately escalated risk situations such as self-harm, suicidal ideation, aggression, absconding, and vulnerability.
• Complete MSE, and clinical risk assessments.
• Adhere to all hospital policies.
• Work collaboratively and cooperatively with all members of the MDT.
• Resolve any workplace conflict in a professional manner through the correct organizational process.
• Maintain confidentiality on all issues relating to the hospital, patients, relatives, and colleagues.
• Identify potential variances and high clinical risk situations, implementing effective management strategies.
• When directed, take charge of the unit by supervising, leading, directing, and motivating staff to achieve outcomes.
• Supervise the performance of nursing procedures to ensure competent, safe care is delivered within the best practice and statutory guidelines.
• Liaise with the nurse unit manager and educators to drive education within the unit.
• Ensure that nursing practices comply with hospital policies, regulatory standards, and accreditation requirements tied to KPIs.

• Assessed patient conditions, monitored behaviours, and updated the treating team with observations and concerns.
• Performed MSE and clinical risk assessments.

• Checked, prepared, and administered medications in accordance with legislation, organizational policies, and procedures.

• Performed as a named nurse within an MDT to a specialised patient group; facilitating alcohol and substance misuse detoxification.
• Completed risk assessments, contributing to risk management in compliance with NHSGGC policy and procedures.
• Working in partnership with patients and caregivers, using values-based practice to formulate and deliver person-centered care, promoting and supporting physical and mental well-being, and preventing relapse.
• When directed, take responsibility for the unit in the absence of the ward manager, or senior charge nurse.
• Collaborated with multidisciplinary teams, including psychiatrists, social workers, physicians, psychologists, community mental health teams, and others, to ensure continuity of care for each patient.
• Completed MSE with patients and responded accordingly to any acute concerns or deteriorations then providing ISOBAR handover to MDT.
• Administered prescribed medications and treatments in compliance with NHSGGC policy, monitoring the effectiveness of prescribed treatments, and managing any unwanted or adverse reactions.
