Senior (Charge) Nurse – Brentwood Aged Care Facility 2007 − 2017
Brentwood is an 84-bed facility in Sydney catering to high-needs residents.
• Managed a team of 9 including a Registered Nurse, and healthcare assistants (HCAs)
• Serviced high-needs residents with multiple co-morbidities, those on palliative care and a number with degenerative diseases such as MS (Multiple Sclerosis) and Parkinson’s Disease requiring infusion pump, and those requiring complex care for PEG/NGT feeding, Tracheostomy, SPC
• Ensured that every resident has a care plan and that the plan is reviewed and updated frequently as the health of the resident deteriorates
• Liaised with doctors, pharmacists and physiotherapists to ensure that care plans are appropriate for the individual and are aligned with best practice
• Managed any emergencies or incidents that arose and determined whether these indicated a change to the care plan was required
• Advocated for the client at all times and ensured that their families fully understood the treatment plan and were able to provide the best support they could
• Focused on preventative action rather than reacting to emergencies − by knowing individual residents and being able to anticipate the course of events
• Reviewed and appraised the work of the HCAs; ensured that the whole team understood the need to provide emotional support as well as clinical care
• Ensured that comprehensive and accurate client information and incident reports were captured in the TRAX system for easy review and auditing
• Facilitated internal audits and took any remedial action required based on the findings
• As the Kirkwood unit RN, provided coordination of nursing and HCA resources across all units
• Responsible for the preparation and maintenance of care plans, had oversight of the work of nurse aides and HCAs and managed any incidents or emergencies
