• Thorough assessment of acute and high risk patients presenting to Northern Health. Implementing safe discharge plans and/or advocating for admission when both social and health issues have been identified.
• Referring to and working within an allied health team to ensure the best outcome for a patient. Currently work closely with Physiotherapists, Social Workers, Occupational Therapists, Dieticians etc

Implementing a safe discharge plan and completing referrals to both internal and external stakeholders as required. These include but are not limited to; HARP (Hospital at Risk Program), ACAS, MAC, Carer Gateway, Home Care Providers/ Case Managers, GP's , RACF (respite and permanent care placements), HITH, district nurse providers, Post Acute Care, Home and Bed Based TCP
• Maintaining an up-to-date knowledge of local and domestic services and programs. Familiarisation with time frames and expected outcomes
• Mentor to new staff members and students rotating throughout the department
