• Delivering pre- and post-release case management, collaborating with prisons, courts, and community corrections to support reintegration.

Intake and Assessment Officer

• Completing intake assessments and developing transition plans within required timeframes.
• Respond to intake calls and referrals in a timely and respectful manner
• Provide relevant information and appropriate referral pathways (either internally or externally), including brief intervention, referrals and secondary consultations.

• Conduct MARAM-aligned risk assessments and develop safety plans

• Handling calls to/from a range of different people including professionals, families and other members of the community.
• Manage the referrals inbox and Central Intake Voicemail, register client referrals on Mastercare, upload clinical documents in a timely manner.
• Draft letters, file documents, answer phone calls and email management.
• Ensure the completeness of referrals and request additional information from referrers as needed.
• Extraction of program and client data and producing tables, graphs and reports as directed to support managers with reporting requirements and analysis.

• Recruitment and intake of carers
• Providing intensive supports that meet legislative requirements and ultimately achieve reunification and long-term stability for children in care
• Providing services to children and young people with complex needs and their kinship or foster care family involved in statutory out of home care
• Participating in assessment processes and ensuring carers and home environments are authorized/approved and remain under active review
• Investigating Standards of Care and HARM Report against carers
• Extensive experience in family violence trauma, women facing Domestic Violence, crisis response and family services
• Tailored intensive case management support to address goals through a range of interventions
• Having an active professional voice at meetings with Child Safety, QLD Health, QLD Education and relevant stakeholder to aid holistic approach for client support

• Coordinate the renewal of approval assessment process and all administrative tasks related to distributing, receiving and lodging documents with Child Safety
• Responding to complex after hours matters supporting our 30 residential units, foster carers and any other person needing support.
• Experience working with young people who have or are experiencing self harming, mental health issues or substance dependency/misuse
• Highly developed skills in providing direct care and undertaking in person risk assessment and responding to and managing complex, high risk behaviour of children and young people
• Extensive experience in the application of relevant theoretical approaches that underpin practice for working with vulnerable children, young people and families.
