In my present Palliative Care AIN role at Silver Chain, I have successfully managed a numerous terminal ill clients and support their families through the complexities of terminal illness. My dedication to client-centered care has been recognized by Silver Chain and Clients.
Support clients nearing the end of their life in the comfort of their own home, ensuring people receive the very best standards of care.
DUTIES:
• Help clients with bathing and showering, dressing, hair and oral care, going to the toilet and supporting bowel care according to provided care plans.
• Change and make hospital beds.
• Pressure area and skin care, repositioning Clients and educate families and carers.
• Attend simple wound dressing.
• Support spiritually and emotionally clients, families and carersduring the end of life.
• Working with Doctors, Registered Nurses and other health care professionals throughout the day
• Maintaining the comfort and dignity to clients.
• Providing company and social support to clients, (respite care).
• Helping clients to dress and undress and applying ted stockings and special garments.
• Observing, reporting and documenting health changes for other medical staff.
• Assisting the OT personnel to develop the exercises plan.

• Fully trained and experience to assist clients with mobility, transferring turning and repositioning bed bound clients for PAC.
• Competency and experience providing basic wound care, including dressing change under supervision of RN.
• Ability to record client observations accurately and report any issues to the team leader.
• Fully trained to follow workplace safety procedures and identify and report hazards in order to contribute with the stablished OSH procedures.
