-Completed comprehensive admissions for new onboarded clients and services under the CHSP Funding Model

-Maintained a Person Centred Approach for all clients to ensure they received services that would allow the client to live their best life and to be able to have full control and independence


-Developed detailed Careplans for each service that the client needed so that the client and careworkers could work autonomously and seamlessly
