- Mentors, develops and supervises service facilitators.
- Through the service facilitators, is responsible for the wider team of support workers.
- Contributes to the delivery of quality of service.
- Promotes a clear vision of quality in services and supports, reviewing and monitoring quality and leading innovation and continuous improvement.
- Works closely with service facilitators, prioritising and matching referrals to the most appropriate service facilitators to ensure the development of self-determination, effective individual plans, service design, continuous review and improvement of quality services is achieved.
- Provides reports internally and externally as required.

- Resolved customer complaints in professional and timely manner.
- Providing high level of customer support to home care package clients and the home care package team.
- Providing timely follow up of referrals and service requests to facilitate optimal care provision for clients to services.
- Scheduling and rostering of services into appropriate systems and documenting all actions and liaisons
- Coordination of appropriate services internally and externally and scheduling of appointments.
- Appropriately seeking care management guidance on potential risks identified when engaging with clients and internal/external.
- Capturing data for service planning and reporting.
- Monitoring email inboxes and website portals for new service requests.
- Liaising with individuals and families to inform them that health Ability has arranged their services as discussed with the home care package team
- Documentation of all communications and actions into appropriate client management systems
- Working within role boundaries and escalating when unsure or out of role
- Support the team to arrange for purchasing of approved goods and services for their clients and day program
- Assist the team with administration tasks

- Personal hygiene, grooming, manicuring and oral hygiene.
- Continence management.
- Communicates with all residents effectively including the maintenance and the fitting of hearing aids and glasses as required.
- Assisting those residents’/clients with challenging behaviours, by participation in the development and implementation of an individually designed management plan.
- The residents’/clients mobility, by assisting with walking, use of wheelchairs and other aids and transfers.
- Supports and assists residents with food and fluids as required.
- Maximises the residents’/clients physical function by assisting with rehabilitative treatment such as physiotherapy exercises.
- Assists the nursing staff with treatments as specified
- Assists the nursing staff by observing and reporting promptly any changes in the condition or well being of residents.
- Ensure the residents privacy, dignity and respect are maintained at all times.
- Ensure that no residents experiences isolation and loneliness unless determined by choice.
