Coordinating individualized discharge plans to manage safe transition back into community and home environments.
Develop individual treatment plans and provide emotional support to clients.
Support individuals with varying disabilities to develop personal care plans specific
to their needs and goals.
Encourage strong relationships with the families and other careers within the client’s team to ensure open communication and any issues were dealt with in a prompt manner.
Participate and assist with coordinating the other professionals within the multi-disciplinary team to ensure seamless service provision for the clients regardless of the challenges to implementation.

Implement and update the care plans for individuals with complex physical and psychosocial needs to best utilize their limited funding through low-cost/no-cost solutions /community outreach programs and targeted outreach programmes.
Maintain high standards of personal hygiene and professional dress.
Understand and practice Occupational Health and Safety (OH&S) and building safety procedures.
Attend meetings and training as required.
Address customer complaints promptly and satisfactorily.
Maintain courteous, friendly, and professional relationships with clients, families, co-workers, and community partners.
