• Medication training Assisting client with their daily activities Updating clients and supporting with their routine.
• Drove clients safely to social activities and appointments.

Duties include

• Working within the plan of care under the supervision of the RN
• Assist in the delivery of the following direct activities as allocated by a RN
• Document all patient response to direct care/ assistances given include in progress note and clinical notes in accordance with plan.
• Answered incoming phone calls to process requests, transfer calls, or relay messages to appropriate personnel.

Duties include

• Facilitated peer support groups to help clients connect with others.
• Disability support worker
• Assisting with medication and supporting them to achieve their goals
• Updating client health care plans using an computer Working with all range of disabilities including non-verbal and deaf.
• Maintained detailed records of client progress, interventions, and outcomes to ensure continuity of care across service providers.
