Transition Worker

PRINCIPLE DUTIES: •

To deliver casework services to clients of the Transition Program to assist in achieving independent living and access to mainstream and Indigenous support services.

• To work with the children, families and communities of clients where appropriate to enable the provision of support to clients.

• To work in a culturally respectful way with Indigenous clients of the service, their families and communities. • To receive and assess referrals from a range of referral agencies including Corrective Services NSW and ADHC Community Justice Program in conjunction with the Team Leader.

• To conduct risk and need assessments of clients pre-release and in the community to establish long term needs. • To visit clients in prison prior to release with the aim of establishing a working relationship with them.

• To attend case planning meetings with CRC staff, clients of the service and other relevant agency staff to develop an Individual Support Plan.

• To liaise with housing providers, real estate agents and families re the availability of appropriate accommodation. • To liaise and network with relevant community, government and non-government organisations to ensure appropriate referral sources for clients.

• To assist with the organisation and movement of client property. • To liaise and network with relevant community, government and non-government organisations to ensure appropriate referral sources and increase access to existing services. • To provide information to key government and non-government organisations re the issues facing offenders post release in order to increase access to services for the client group

. • To stay informed of research and the development of best practice models of service delivery in the areas of disability and offending behaviour. • To provide content contributions for CRC publications and communications, such as the Annual Report, website and brochures, as required. • To ensure CRC policies and procedures are adhered to at all times. • To take reasonable care for their own health and safety and the health and safety of others, and ensure they comply with any reasonable instruction by CRC management.
