  • The Community Chronic Conditions Service (CCCS) aims to prevent morbidity associated with chronic disease, prevent hospital admissions, reduce length of stay and prevent premature admission to Residential Aged Care
  • This was a new role for the team, and I as required to develop a model of care for psychology, referral pathways and eligibility criteria for the service
  • As the Senior Clinical Psychologist in this multidisciplinary team my role was to high level Psychology clinical services and leadership across the Sunshine Coast Hospital and Health Service
  • This service was delivered via telehealth, home-based and clinic-based appointments
  • I Participated in care reviews and provided clinical supervision to psychology students
  • I also developed and delivered education presentations to Respiratory and Cardiac Rehab patient groups.
