Transitional Housing Model - Case Manager

  • Create and execute a comprehensive, client-centered wrap-around care plan with clear goals focused on transitioning to long-term housing. Continuously monitor progress and adjust the plan based on the evolving needs of the client.
  • Provide onsite support to young people accommodated in the program assisting in developing interdependent, sustainable life options, with a particular focus on developing skills for budget, employment, education and training, connections with community, personal and social development.
  • Uphold client's rights and responsibilities as developed by Hope Street including advocating on behalf of clients within a multi-disciplinary team approach in relevant client, program, and external meetings.
  • Maintain a safe and secure environment for young people, staff, and visitors Community Development.
  • Establish and maintain effective working relationships with key homelessness and allied support services that will enhance client connections to those services and client outcomes.
  • Undertake community capacity-building activities in the region to improve service systems responses to vulnerable young people experiencing homelessness or at risk of experiencing homelessness.

  • Provide education on constructive conflict management and resolution techniques, to avoid unnecessary escalation and aim for a positive outcome.
  • Manage high-complexity youth, supporting them in reconnecting with loved ones and engaging in healthy lifestyle alternatives to address undesirable behaviours. Develop improved communication skills, build their identity, and find a meaningful place within their community.
  • Conduct individual and group counselling sessions to help young people address issues such as family trauma, substance abuse, sexual abuse, criminal behaviour, mental health, disabilities, etc. Establish healthy boundaries and challenge inappropriate behaviour.

  • Amend and manage the ‘Way Out There’ program including the implementation, monitoring, and reporting of compliance requirements such as Risk Management and OHS policies.
  • Work closely with young people to determine their future goals and provide ongoing guidance and support toward their achievement, develop better social interaction skills for improved engagement in the community including constructive conflict management and resolution techniques.
