Transitional Housing Model - Case Manager

- Create and execute a comprehensive, client-centered wrap-around care plan with clear goals focused on transitioning to long-term housing. Continuously monitor progress and adjust the plan based on the evolving needs of the client.
- Provide onsite support to young people accommodated in the program assisting in developing interdependent, sustainable life options, with a particular focus on developing skills for budget, employment, education and training, connections with community, personal and social development.
- Uphold client's rights and responsibilities as developed by Hope Street including advocating on behalf of clients within a multi-disciplinary team approach in relevant client, program, and external meetings.
- Maintain a safe and secure environment for young people, staff, and visitors Community Development.
