An experienced advanced critical care nurse of 20 years with 15 years experience working at the Princess Alexandra Hospital, predominantly in the Emergency Department and also in a variety of departments and roles all within the critical care umbrella, I am able to demonstrate an advanced application of knowledge and contemporary practice and theory in this specialty area. I am flexible and adaptable, efficient in high pressure situations, and very comfortable working in an unpredictable environment. I am familiar with prioritizing competing complex patient related tasks. I am currently working as a Clinical Nurse in the Intensive Care Outreach Service, and also a Clinical Nurse Consultant in the Patient Access Coordination Hub, (PACH) and also the Inter Hospital (IHT) Nurse Manager Role for Metro South.

The purpose of the MSH IHT NM is to provide expertise in patient centric centralised, coordinated and clinically safe transfers of patients into and out of MSH. The MSH IHT NM will negotiate, consult and communicate to enhance and coordinate optimal patient flow.
The NM works collaboration with MSH PACH and the wider health care team to support the Health Service in
the provision of safe quality healthcare through exemplary clinical nursing standards of practice.
MSH IHT NM uses real-time data from MSH hospitals and QAS to gain insight into what is happening across the
Metro South Health Service. This information is used to:
-Facilitate the timely referral acceptance and transfer of the patient to the most suitable facility.
-Analyse and respond to capacity issues.
- Contact point for escalation for MSH to external Health Services.
-Facilitate direct transfers which support appropriate sharing between departments, hospitals and external
providers to ensure patients get access to the right care at the right time in the right place.
- Liaise with QAS, Patient Flow Units, Additional Beds Nurse Manager and MSH PACH CNC regarding the timing
of transfers.
-The Intensive Care Unit (ICU) Outreach Service, in alignment with standard 8 of NSQHS, provides expert nursing practice in collaboration with the multidisciplinary team, to provide and maintain a continuum of care for patients discharged from the ICU and those at risk of clinical deterioration in the ward environment.
-Provide clinical leadership and mentoring for staff and other members of the health team across the continum to ensure effective and supportive care for all patients, relatives and staff.
-Participate in the assessment, implementation, evaluation and recommendations of systems/services/practices within the are of medical and surgical nursing to ensure safe practices related to patient deterioration.
-Use hospital based processes and procedures to monitor and assess patient deterioration and use established escalation systems within the hospital to notify of deterioration.
-Provide clinical expertise, consultancy and education interdisciplinary, informal and formal, to staff in the PAH to facilitate them to provide safe clinical nursing services to patients.
-Review patients discharged from ICU to the ward and follow up with MET calls as determined by clinical condition.
-Review patients referred to service from ED, PACU, Ward Staff, Allied Health, Medical Staff and AHNM across all specialties on PAH campus.
-Undertake Ryan's Rule calls and make Independent clinical reviews, referring and escalating patients to appropriate multidisciplinary teams.
-Attend and assist at all Medical Emergency calls and code blue calls within the entire PAH campus and assist with all Advanced Life Support and admission of the patient to ED or ICU if required. If the patient remains in the ward environment ensure an appropriate management plan has been implemented and a safe and appropriate eplan is in place.
-Provide a continum of care and bed side assessment for patients who have been discharged from ICU, Post MET follow up and assessment, and all patients who are a direct referral (from all areas) at PAH.
-Assist in clinical facilitation and nursing education to promote a learning culture by encouraging reflection and professional development, providing clinical teaching, in-service education, and assisting/supporting others to maintain portfolios/records of learning.
- Managed and organized the simulation educations service in the Emergency Department.
- Responsible for undertaking BLS and ALS training and assessment for Nursing and Medical staff.
- Responsible for simulation education training for undergraduate and graduate nurses within the emergency department.
- Responsible for creating engaging, real life, evidence based simulation scenarios.
- Professionally debrief staff post all simulation sessions.
-Responsible for having the ED SIm Room stocked and functional for simulations. Ordering of stock, and ensuring new equipment or processes were applied.
