Summary

An experienced advanced critical care nurse of 20 years with 15 years experience working at the Princess Alexandra Hospital, predominantly in the Emergency Department and also in a variety of departments and roles all within the critical care umbrella, I am able to demonstrate an advanced application of knowledge and contemporary practice and theory in this specialty area. I am flexible and adaptable, efficient in high pressure situations, and very comfortable working in an unpredictable environment. The purpose of the MSH IHT NM is to provide expertise in patient centric centralised, coordinated and clinically safe transfers of patients into and out of MSH. The MSH IHT NM will negotiate, consult and communicate to enhance and coordinate optimal patient flow.
The NM works collaboration with MSH PACH and the wider health care team to support the Health Service in
the provision of safe quality healthcare through exemplary clinical nursing standards of practice.

MSH IHT NM uses real-time data from MSH hospitals and QAS to gain insight into what is happening across the
Metro South Health Service. This information is used to:

-Facilitate the timely referral acceptance and transfer of the patient to the most suitable facility.
-Analyse and respond to capacity issues.
- Contact point for escalation for MSH to external Health Services.
-Facilitate direct transfers which support appropriate sharing between departments, hospitals and external

  • MSH PACH operates as a command and control centre that gives a birds-eye view of demand and the flow of patients across our network of five hospitals. PACH is designed to help alleviate the pressure on the region’s ambulance services, emergency departments and hospitals. MSH PACH is staffed with Metro South Health clinicians and Queensland Ambulance Service (QAS) personnel who work together to support patient flow, manage the increasing demand for beds, and help patients access the right care at the right time.
  • The purpose of this role is to provide clinical expertise and consultancy through advanced clinical leadership and decision making relevant to the speciality of Emergency Nursing, Patient Flow and Hospital Avoidance. The MSH PACH Clinical Nurse Consultant (MSH PACH CNC) provides direct and indirect care as required and works in collaboration with the wider health care team to support the clinical unit in the provision of safe quality healthcare through exemplary clinical nursing standards of practice.
  • The MSH PACH CNC uses real-time data from MSH hospitals and QAS to provide oversight into what is happening across the Metro South region. This information is used to:
    ▪ Manage surges in demand.
    ▪ Analyse and respond to capacity issues.
    ▪ Monitor and effect support for facilities during times of high demand on services ensuring safety of patients and staff.
    ▪ Facilitate and direct load sharing between departments, hospitals and external providers to ensure patients get access to the right care at the right time.
    ▪ Identify alternatives to hospital where possible.
  • Guide and support staff in delivering appropriate patient care and service delivery to ensure effective flow
  • Actively monitor Emergency Department (ED) Length of Stay (ELOS) s and take action to improve the flow of patients through the MSH Emergency Departments.
  • Liaise with Patient Flow and ED staff across MSH to identify any potential barriers to prevent the achievement of ELOS.
  • Identify areas of crisis (where there are capacity issues, surge activity or severe access block) and respond appropriately.
  • Coordinate all non-time critical Inter Hospital Transfers (IHT) from 1700 – 0100 and 0630 - 0830 when the MSH IHT nurse completes their shift.
  • In collaboration with QAS, provide advice and direction regarding appropriate allocation and triage of Medically Authorised Transfers requests.
  • Liaise with the Queensland Patient Access Coordination Hub to advocate for MSH and to offer assistance to other Hospital and Health Services when able.

-The Intensive Care Unit (ICU) Outreach Service, in alignment with standard 8 of NSQHS, provides expert nursing practice in collaboration with the multidisciplinary team, to provide and maintain a continuum of care for patients discharged from the ICU and those at risk of clinical deterioration in the ward environment.

-Provide clinical leadership and mentoring for staff and other members of the health team across the continum to ensure effective and supportive care for all patients, relatives and staff.

-Participate in the assessment, implementation, evaluation and recommendations of systems/services/practices within the are of medical and surgical nursing to ensure safe practices related to patient deterioration.

-Use hospital based processes and procedures to monitor and assess patient deterioration and use established escalation systems within the hospital to notify of deterioration.

-Provide clinical expertise, consultancy and education interdisciplinary, informal and formal, to staff in the PAH to facilitate them to provide safe clinical nursing services to patients.

-Review patients discharged from ICU to the ward and follow up with MET calls as determined by clinical condition.

-Review patients referred to service from ED, PACU, Ward Staff, Allied Health, Medical Staff and AHNM across all specialties on PAH campus.

-Undertake Ryan's Rule calls and make Independent clinical reviews, referring and escalating patients to appropriate multidisciplinary teams.

-Attend and assist at all Medical Emergency calls and code blue calls within the entire PAH campus and assist with all Advanced Life Support and admission of the patient to ED or ICU if required. If the patient remains in the ward environment ensure an appropriate management plan has been implemented and a safe and appropriate eplan is in place.

-Provide a continum of care and bed side assessment for patients who have been discharged from ICU, Post MET follow up and assessment, and all patients who are a direct referral (from all areas) at PAH.

-Assist in clinical facilitation and nursing education to promote a learning culture by encouraging reflection and professional development, providing clinical teaching, in-service education, and assisting/supporting others to maintain portfolios/records of learning.
