  • Executing and delivering thorough reporting and handover requirements on every shift.

From Jan 2020 - August 2020 - I worked in a Transitional High Needs placement home (THN - Model) of OOHC - Out-of-Home-Care.

High/complex needs disability placement:

Supporting and caring for a young person with significantly complex disability needs. We were a 'live-in' 24hr double staffed team on a rotating roster. This young person was 14 years of age when he transitioned into our care. He was born with Cornelia De Lange syndrome, on the Autism spectrum, inclusive of ADHD, mental health dysregulation and trauma history and was mostly non-verbal. Supporting this young person required Carers to wear protective gear, chest guard and protective full arm bite gloves. Carers assisted with everyday routines such as personal care (showering, toileting, dressing), social and recreational activities, community access, medical appointments, school run, meal preparation, domestic duties and emotional support.

This was the most challenging position I have ever undertaken but also the most rewarding and I built a meaningful rapport with this young person.

Out-of-Home-Care - SFC (Specialised Foster Care)

Caring for young person on a one-to-one basis. These types of placement models are for individuals who do not fare well in the FGH dynamics and who require specialised and higher levels of emotional support. Trauma based therapeutic care model.
