The ICU Liaison Nurse is an advanced practice nurse who works in a consultancy role with an expertise in critical care nursing and significant experience.
The Liaison Nurse acts as a resource for ward staff and receives referrals from medical, nursing and allied health staff to assist in the management of patients with complex needs on the wards.
Provide support for ward staff in the delivery of care.
Act as a bridge to transfer from ICU to the wards. Review and facilitate the discharge of patients from ICU. Ensures patient discharge is timely and efficient and prevents unplanned readmission to ICU.
Follow up patients post ICU discharge. Supports patients and relatives through the transitional period from ICU to ward and home.
Maintain a continuum of care by a follow up program on the wards and reducing hospital LOS.
Ensure that CVCs are removed in a timely manner and monitored to reduce line infection rates.
Provide education and clinical support to ward staff regarding patient care issues.
Co-ordinate and facilitate the management of all patients with a tracheostomy at Barwon Health. Involved in the development of a Tracheostomy Education Package for ward staff and the initiation of Tracheostomy Emergency and transport bags.
Receive referrals for patients requiring TPN and books long-term line insertions. Reviews TPN patients on a daily basis and monitors fluid, glucose and electrolyte status.
Attend MET and Code Blue response calls with the ICU Registrar or Consultant.
Manage critically ill patients on the ward and initiates treatment and therapy until an ICU or CCU bed becomes available.
ICU Liaison Nurses, effectively avoiding an ICU admission, can manage patients who traditionally would require high-level care on the ward.
Patients in acute respiratory distress requiring CPAP or non-invasive ventilation can be managed on the ward until more stable. Electrolyte replacement requiring monitoring during administration can be performed with portable monitoring equipment and the presence of the ICU Liaison Nurse.
Identify patients whose condition has the potential to deteriorate and ensures special clinical needs are met. Make referrals to the patients treating team, allied health or to the ICU team as appropriate.
Review patients who's treating team has requested ICU care in EMD, Recovery or on the wards and with the ICU Registrar or Consultant decides on appropriate level of care required.
Development of an ICULN database to monitor quality of care. Involved in nursing research projects and presentation at ANZICS conferences to colleagues. The ICU Liaison Service aims to ensure all staff use best practice initiatives by working in a consultant role to act as a clinical resource to all staff. This is also achieved by the service:
Maintaining a database on all patients referred to the service and interventions performed for data collection purposes.
Actively participating as a member of the Resuscitation Committee.
Assisting in formulation and review of standards, procedures and policies that support the provision of safe patient treatment and care.
Participating in ongoing review and analysis of the service's practices and the impact of new directions on the service.
Participating in the development of annual business plan for the ICU Liaison Nurse service.
Contributing to research relevant to the provision of the ICU Liaison Nurse Service. The Liaison Nurses have surveyed ward staff to determine their perceptions and satisfaction with the service with the result being overwhelmingly positive. In collaboration with three other public hospitals, the results of this survey were accepted as a poster presentation, entitled “Intensive care unit Liaison Nurse service is highly valued by ward nursing staff” at ANZICS/ACCCN ASM in Rotorua, New Zealand in 2007
Development of an Oxygen and Humidification education package.
Roll out and education to staff of a new oxygen delivery system at Barwon Health.
Involved in the provision of MET in-services to all new staff at Barwon Health. Providing clinical skills education to Junior Medical Officers.
ICU Liaison Nurses have completed the Respecting Patient Choices Program and are able to initiate discussions with patients regarding treatment limitations. In many instances, the initiation of these discussions and onward referral to RPC co-ordinators or relevant Medical Officers has seen patients being managed palliatively on the ward, rather than being unnecessarily admitted to ICU for inappropriate or possibly unwanted medical interventions, which may cause undue distress to patients and their families
