USIN Responsibilities:
**Shift Planning and Prioritization: I developed and implemented effective shift plans, ensuring consistent task prioritization and adherence to departmental guidelines. I also always made myself aware of the patient's ARP ( Acute Resuscitation Plan) plan.
**Comprehensive Patient Assessment: Conducted thorough head-to-toe assessments, collecting objective and subjective data. This included neurological assessments (Glasgow Coma Scale, 4 AT), respiratory assessments (including auscultation), neurovascular assessments (upper and lower extremities), abdominal assessments, vital signs monitoring, pain assessments, wound assessments, ECG interpretation, and bladder scans.
**Early Identification of Patient Deterioration: Proactively identified and responded to patient deterioration by monitoring patient status, vital signs, pathology reports, and multidisciplinary team notes.
**Effective Communication and Collaboration: Maintained clear and timely communication with primary nurses, team leaders, nurse unit managers, treatment teams, wound care specialists, stoma nurses, and other multidisciplinary professionals to ensure optimal patient care.
**Meticulous Documentation: Ensured accurate and timely documentation of patient progress notes and all relevant charts, including skin inspection records and fluid balance charts.
**Behavioural De-escalation: Applied safe and effective nursing interventions to de-escalate behavioural issues in patients with dementia, delirium, alcohol withdrawal (ETOH), and mental health conditions. Interventions included engaging patients in activities such as colouring, reading, storytelling, implementing the SUNFLOWER tool, and providing companionship through walks and other activities.
During my two-year tenure as a USIN at Gold Coast University Hospital(GCUH) and Robina Hospital, I gained extensive experience across a diverse range of wards, including neurological, cardiac, respiratory, general medical, medical decision unit, emergency department, immunology, complex clinical care, elderly care, orthopaedic, and mental health units. This broad exposure provided a comprehensive understanding of patient care across various specialities.
My experience working with older adult patients (5+ years) and post-operative patients (precisely those undergoing neck of femur surgery) has honed my ability to anticipate and manage potential complications. I am adept at identifying and mitigating risks associated with post-operative pain management, pressure injury prevention, and overall physical decline in elderly patients. My proactive approach includes implementing preventative measures such as regular repositioning, meticulous pressure area care, frequent pain assessments, proactive pressure injury assessment, application of barrier creams, appropriate stocking application (following consultation with the treatment team), ensuring adequate fluid intake, encouraging food intake, and vigilant monitoring of PIVCs for signs of phlebitis.
I am confident that my two years of experience as a USIN and my five years of experience in aged care have fully prepared me for the role of Registered Nurse. My skills encompass a holistic approach to patient care, combining experience, foresight, clinical intuition, and a strong foundation of nursing knowledge.
