Commonwealth Home Support Program (CHSP), Home Care Packages (HCP), Veterans' Home Care (VHC) Coordinator.

Coordinating service for the above programs through the community.

Our office covers from the Barossa, Light region, Gawler & surrounding areas, through to Mt Pleasant, Yorke Peninsula.

Key Responsibilities

  • Coordinate in home care services from domestic, social support, coordinate OT assessments and purchase of equipment for HCP customers, liaise with clinical teak to seek advise when clinical concerns are identified, garden and home maintenance, transport, liaise with allied health professionals, whilst working within the aged care guidelines and Uniting Communities policies and procedures.
  • Conduct initial assessment's & reviews for customer receiving services for the above programs.
  • Travel to remote and rural areas.
  • Respond to client complaints with a solution focused approach and ensure complaints are documented and are resolved in a timely manor.
  • Rostering of services, with the approach of matching the customer to best care worker that is suited, not only experience or availability but also their personalities.
  • Work with the customer, customer family/carers/representatives, service providers, Government bodies to ensure customers access the correct level of support it is necessary and the best supports according to each individual customer.
  • Ensure all documentation is relevant , completed and uploaded to a customers file, regularly visiting to ensure they are receiving the best service for that individual.
  • Complete all mandatory training.

Care Workers

  • Working with care workers across the above regions. providing mentoring, training (as per UC guidelines), IT support, conducting staff and supervision, compliance documentation, meeting support - Creating agenda following Uniting Communities standards, incorporating training, news, compliance, WHS and everyday items that may be relevant to the care workers.

  • Working alongside out IT department in Metro

Compliance
