Summary

Over the past six years, I have been lucky enough to travel around Australia and work across a variety of roles and clinical settings, from tertiary hospitals to community services. These experiences have been invaluable and have highlighted the importance of fostering a team culture which recognises and values diversity, as to best deliver patient-centred care. My previous roles have involved leadership, management and working across multi-disciplinary teams and healthcare continuum in rural, remote and indigenous settings.

Work History

HP4/3 Discharge Transit Lounge Pharmacist

  • Provide clinical leadership within the Discharge Transit Centre multi-disciplinary team using highly developed knowledge, skills and experience in the provision of Pharmacy services.
  • Provide an efficient and patient-centred clinical pharmacy service to patients accessing the Discharge Transit Centre
  • Work cooperatively and effectively with the multidisciplinary team within Pharmacy, the Discharge Transit Centre team, differing treating teams, and with patients/carers.
  • Support the delivery of operational Pharmacy functions including dispensing, distribution and procurement of medicines.
  • Provide professional clinical support to Pharmacists, Pharmacy Interns, Students and Clinical Assistants, with an emphasis on safe medicines practice, and timely and efficient distribution to improve patient flow.
  • Collaborate with the Discharge Transit Centre pharmacy team to ensure effective cooperation within and across the work unit to enhance service delivery, quality outcomes and patient services.

  • Provide independent, expert clinical pharmacy services, autonomously applying critical and complex decision making to patient care – including solo remote trips to hospitals within the service.
  • Pharmacy Management Group involvement facilitating planning and evaluation of pharmacy service activities, engaging stakeholders, and delivering solutions e.g. critical drug shortage list for monitoring in line with Pandemic Plan, rural AMS committee / service continuity within the Central West Health Service.

HP4 Specialist Pharmacist – Oncology/Haematology

  • Sole Cancer Pharmacist in Central Australia (inpatient, outpatient, clinics in communities, rural hospitals).
  • Managed a pharmacy technician and provided support to nursing staff, cancer care coordinator and junior medical officers – given no consultants in either specialty on site.
  • Provided a specialist cancer service and managed cancer patients which involved attending telehealth appointments, participating in weekly multidisciplinary meetings, attending weekly state-wide haematology meetings where I presented our haematology patients given the lack of a junior medical officer and assisted nurses with all queries in regard to bloods, infusion rates/reactions, and toxicities.
  • Provided expert advice to assist with improved management of chemotherapy contracts with external providers, produce monthly expenditure reports, apply comprehensive understanding of the PBS and how to access non-PBS medications, eg compassionate access programs.
  • Collaboration with Oncologists, Haematologists and all Specialists in Day Infusion Unit either in person or via telehealth.

Student Preceptor Portfolio

  • Site preceptor for all students and universities. Coordinated timing of placements, student rosters, presentation topics/project work, student/staff/university feedback and ensured students met placement requirements.

Senior ED/ICU preceptor for rotating pharmacists, Residency Program Mentor (ED/ICU)

  • Attended ED and ICU advisory committee meetings, reviewed imprest, organised and undertook ClinCATs/Mini-CEX on resident and non-resident pharmacists, assisted with education, provided clinical support and assited with organising complex medication supply and therapeutic drug monitoring.
  • Undergo regular performance management of self (3-6 monthly ClinCATs).

Pharmacy Leadership Team

  • Attended weekly meetings, discussed HP3 rotation allocations, residency requirements, education roster, student roster, reviewed leave requirements and adjusted roster accordingly.
  • Participated in weekend and OnCall roster and ensured was contactable for emergency duties after hours relating t oncology/haematology/ED/ICU queries.
  • Tennant Creek roster participant – sole pharmacist.

HP4 Clinical Pharmacist, PPMC Navigator

  • Partnered Pharmacist Medication Charting Navigator - Credentialed Pharmacist (OSCEs with Head of General Medicine and Emergency Department).
  • Triage patients who meet criteria and accept referrals from General Medicine Registrar and ED Registrar.
  • Navigate team of PPMC pharmacists and allocate patients and ensure equipped for charting on time with registrar. Provide clinical support to charting pharmacists.
  • Actively track and document charting to achieve KPIs to secure funding for ongoing service.
  • Participate in after-hours roster and OnCall roster.

Specialist Inpatient Oncology and Haematology Pharmacist Backfill.

  • Organised complex inpatient chemotherapy, managed toxicities and facilitated in house manufacturing or external ordering of emergency chemotherapy.
  • Attended weekly multidisciplinary meetings with Oncology, Haematology and Palliative Care teams.
  • Facilitated complex discharge planning and counselling of complicated regimens and end-of-life medications.
  • Provided support to HP3 rotating pharmacist.
