• Clinical leadership to the unit and coordination of patient care for the home dialysis population in Southern Sector of HNEAHS (190 patients)
• Act as a resource to the JHH Department of Nephrology, generalist nurses, general practitioners and allied health staff in the delivery of care
• Provide high quality nursing assessment, planning, implementation and evaluation using best practice principles as part of a collaborative partnership with patients, families, the multidisciplinary team and other key stakeholders
• This is in accordance with the relevant legislations, organizational policies and procedures, professional standards of practice
• Lead research and quality activities including service planning and development, clinical audit, clinical redesign, practice development projects and research
• Assist with the formulation and evaluation of policies and procedures
• Liaise with Medical Practitioners, Allied Health staff, Pre-Dialysis Clinical Co-ordinator, Patient flow coordinator and other Team Leaders to implement patient management plans e.g
• Suitability to commence in community facility
• Development and contribution to a collaborative team environment that focused toward patient centred care
• Provide appropriate generalist and specialist education, health promotion and prevention relating to acute care nursing issues, for nursing students, colleagues, patients, families and other interested groups or individuals to enhance understanding and knowledge about relevant illness and disease processes
• Assists with the unit budget and cost containment by maintaining an awareness of financial constraints and by utilizing resources effectively and wisely

• Coordination of home PD Training which involves assessment of PD suitability, education, training and organizing supplies
• Personally, involved in commissioning monthly PD Clinic, to review patients more frequently to avoid hospitalization at zero cost to Wansey
• Currently maintaining 119 patients at home, with 3 different automated and manual Peritoneal Dialysis Systems (one of largest in Australia)
• Provide consultation and educational services to nursing homes, regional hospitals, private hospitals and support PD patients at JHH
• Involvement in international multicenter trial of new dialysis fluid for better preservation of Peritoneal membrane
• Delivering Dialysis Prescriptions to patients to ensure that they are adhering to national and international targets
• Peritonitis management, keeping statistic, retraining at home, problem solving over the phone, fluid assessment is part of daily duties
• All other duties involve in smooth running of a unit

• Assessment of general condition of patients i.e
• Dry weight, blood pressure, fistula care, exit site care, diet and medication assessment
• Problem solving and emergency procedure reviews in the unit and at home to maintain patient at home
• Evaluate social needs of patients and liaise with other health care professionals accordingly
• Provide education to patients, carers, consultation, act as a resource person, retraining at home, in services and seminars
• Participate in community awareness programmers and provide education for community staff
• Demonstrate effective communication skills with every significant other caring for a home patient
• Assist with formulation, implementation, monitoring and evaluation of service provided
• Actively participate in quality improvement
• Pre dialysis home environment assessment for suitability of home dialysis
• Be aware of the hospital fire, disaster and other emergency procedures
• Documentation and statistic as required by nursing administration
