-Support patients to participate in age appropriate activities and tasks including employment, in home support and community access.
-Support patients to access activities and outings in the community.
-Provide direct assistance to each patient in accordance with their service plan and as directed
-Work in accordance with the medication administration policy, work health and safety procedures and patients right's policies including privacy and confidentiality.
-Monitor the safety and well-being of the patients and report any concerns to the Manager.
-Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in patients notes and communication book as required.
-Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
-Work with patients, family/ carers, relevant case managers and other professionals to support the patient's individual goals.
-In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
-Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
-Assume the role of key worker for designated patients, including the preparation of a monthly report on patient's progress.
-Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
-Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
-Provide back up and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Report any quality and safety incident or hazards at work to their Manager or Supervisor.
-Carry out their roles and responsibilities as detailed in the relevant policies and processes.
Personal Hygiene: Assisting with personal hygiene, including showering, bathing, and dressing.
Mobility: Helping patients with mobility and transferring between beds, wheelchairs, etc.
Communication and Support: Providing comfort and emotional support to patients.
Monitoring: Monitoring patient health and reporting any changes or concerns to nurses.
Patient Observation: Observing and reporting changes in patients' condition, behavior, and needs.
Medication Administration: Assisting with medication administration under the supervision of a registered nurse.
