• Conducted comprehensive mental health assessments, gathering relevant patient history and collaborating with interdisciplinary teams to formulate individualized care plans. Completed all my patients' MSE and handover during each shift.
• Once discovered abnormal vital signs in a patient, QADDS score was 4, suspected medication intake; immediately notified the doctor.
• Provided empathetic and compassionate care to patients experiencing a range of mental health disorders, fostering a therapeutic environment conducive to healing.
  

Cardiac Ward Placement Experience

• Wound Dressing Changes:

Proficiently performed wound dressing changes as part of the patient care routine. Ensured adherence to infection control protocols and maintained a sterile environment during procedures.

During this placement, I personally experienced the passing away of a palliative patient during my shift for the first time. I learned how to handle and face extremely grief-stricken family members, and the importance of providing patients with respect and dignity.
