• Identifying and reporting risks and hazards by completing incident forms available online accordingly or by submitting EMPAC requests to remove or fix hazards or risks.
• Investigate incidents and hazards and document findings as the Occupational Health & Safety Representative.
• in order to continuing professional development to ensure continuous registration by engaging in workshops, education sessions and performing quizzes where applicable.

• Liaise with the allied health team including physiotherapists and assistants, occupational therapists and lifestyle team, Clinical Nurses, Registered and Enrolled Nurses, personal care assistants by using well developed communication skills, keen observation and monitoring of patients and by accurate and concise documentation.
• Follow up with medication changes after practitioners' reviews and post discharge: ensuring medication changes are made aware to residents, families and staff; ensuring medication changes are correct and updated on computer system; monitor effectiveness of medication via assessment, charts and documentation.
• Administered medications via oral, subcutaneous injections and intramuscular injections and monitored responses.
• Oversaw delivery of care for patients on from hostel care to specialized Dementia wards, providing direct care, delegating tasks and assignments to personal care assistants (PCA), and supervising team comprised of PCA, PTA (physio therapy assistants) and LST (life style team) while delivering care to patients.
• Managing and overseeing multiple wards, patients and PCA during shifts. Coordinate with ENs and Upskilled PCA to ensure patients receive the best possible care.
• Delegating tasks to ENs, Upskilled PCAs and PCAs to ensure patients' safety and care needs are met.
• Providing and receiving comprehensive handover to ensure consistent care.

Main duties and key responsibilities:

• Administer vaccination to patients from infants to older adults.
• Documentation of vaccinations administered accordingly.
• Provide COVID-19 vaccination to patients in primary healthcare setting.
• Preparing and setting up COVID-19 vaccines to be administered to patients.
• Providing education to patients and family prior to vaccine administration and inform of possible side effects to be aware of.
• Maintain and manage of stock in clinic from dressings, to equipments used by doctors during health checks to vaccine stocks.
• Sterilisation of instruments.
• Assisting doctors in minor procedures such as minor lesion removal, skin tear repair, IUD removal and insertion, Implanon removal and insertion and cervical assessment.
• Provide comfort and ensuring patients' safety while assisting and setting up minor procedures for doctors.
• Providing simple wound care.
• Obtain vital signs for careplan.
• Setting up for iron infusion, monitoring patients' vitals prior to infusion and post infusion. Monitor for signs of allergic reaction and adjusting rate of infusion according to patients' condition and preference.
• Concise documentation of care.
• Familiarise myself with the system used by clinic, ordering procedure and trouble shooting.
