A four week clinical placement apart of my La Trobe return to practice course . Placed on a general surgical ward, PACU and day surgery.

Patient case mix includes Acute Myocardial Infarction, diagnostic and interventional cardiology, arrhythmia management, cardiac/thoracic surgery, acute heart failure including post heart transplantation.

Out of hospital community healthcare.Provides Neurological rehabilitation, specialising in acute strokes, Multiple Sclerosis, Parkinson's, Traumatic Brain Injury and Motor Neurone Disease.

Responsibilities

• To prevent hospital admissions and provide early supported discharge through triaging of referral's from GP's and hospitals in a timely and organised manner.
• Final decision maker on all referrals and their urgency to be reviewed.
• Managing an extremely busy SPOR (single point of referral) Desk and colleagues.
• Liaising with GP's and acute hospital's as a front line service for the team.
• Training of all new staff members to the SPOR desk.
• Liaise with all clients regarding concerns and review periods.
