  • To prevent hospital admissions and provide early supported discharge through triaging of referral's from GP's and hospitals in a timely and organised manner.
  • Final decision maker on all referrals and their urgency to be reviewed.
  • Managing an extremely busy SPOR (single point of referral) Desk and colleagues.
  • Liaising with GP's and acute hospital's as a front line service for the team.
  • Training of all new staff members to the SPOR desk.
  • Liaise with all clients regarding concerns and review periods.
