  • Complete accurate clinical notes for all consumers and ensure up-to-date and comprehensive clinical records are maintained.
  • Effectively manage a caseload of up to 15 consumers at any given time.
  • Conducting risk assessments and safety planning with consumers who are experiencing suicide ideation or are experiencing domestic violence.
  • Attend and participate in daily and weekly ward meetings to review, discuss, and advocate for consumers
  • Clearly communicate to consumers, their families and the multidisciplinary team via written and verbal communication skills.
  • Providing crisis support/intervention in emergency situations.
  • Organising and attending family and stakeholder meetings to provide updates and support to families and service providers.
  • Providing information and resources to consumers and their families to assist and educate them after their hospital admission, such as community mental health services, carer support services and other community support services such as NDIS or My Aged Care (MAC)
  • Plan, organise and facilitate group activities for consumers that focus on discharge planning and goal setting.
  • Working with and utilising my Social Work skills and knowledge, the Code of Ethics, and the Mental Health Practice Standards to better improve my abilities in supporting consumers and their families during their mental health admission.
