Caseload management of 55+ NDIS participants of various ages and disabilities. Case load consists of clients engaged with the Complex Needs Care Team under NDIA
• Conducted initial assessments and ongoing reviews to determine clients' eligibility and support needs under NDIS guidelines.
• Collaborating with clients and their families to develop personalised support plans, setting achievable goals and identifying appropriate services and resources.
