Renal Dialysis Unit | Sunshine Coast University Hospital | July 2024 (4 Weeks)

• Worked independently as an active member of the renal dialysis team in providing comprehensive care for chronic and acute dialysis patients within the scope of practice.
• Conducted comprehensive Arteriovenous Fistula assessments and fluid assessments for dialysis patients and completing appropriate documentation.
• Developed confidence in safe preparation and administration of Intravenous Iron Polymaltose and erythropoietin stimulating agents (ESA) as per the protocol and under the direct supervision of RN.
• Involved in administering packed Red Blood Cells products to the dialysis patient and conducted required nursing assessments and documentation before and after the administration as per the protocol.
• Maintained appropriate and timely documentation of Vital signs observation, calculating QADDs score, Haemodialysis parameters during dialysis and escalating the abnormalities to team leader.
• Actively engaged and collaborated with interprofessional teams such as physicians, nephrologists, occupational therapists, wound care nurses, vascular nurses and the nurses from other ward during handover of acute patients for dialysis.
• Developed critical thinking skills and clinical decision-making skills required for the patients with haemodialysis while providing safe and patient centred care.

Medicine On Maple | General Practice Surgery | June 2024 (4 Weeks)

• Actively engaged as a part of multidisciplinary team including general practitioner, and other allied health professionals such as physiotherapist, chiropractor, podiatrist, and dietitian.
• Assisted doctors by preparing equipment, maintaining sterile field with minor surgical procedures including shave and puncture biopsies, abscess drainage, excision of cysts and Venesection.
• Provided patient education regarding management of chronic health conditions, conducting assessments and updating the care plan as per their needs.
• Prioritized and escalated the deteriorating conditions of patient as per the protocol including cardiac emergency and pressure injuries related to diabetic neuropathy.
• Took a lead on undertaking health assessments within my scope of practice and asking feedback from the buddy RNs.

• Calculated, prepared, and administered medication (oral, IV, topical, inhaler, and sub cut) under the direct supervision of RN and clinical facilitator ensuring the three checks and five rights of safe medication administration.
• Worked under the direct supervision of the RN in the development and implementation of nursing care plans, demonstrating thorough observation and investigation skills.
• Provided safe and effective care to patients using an individualised holistic approach related to the goals of care and within Professional role limits.
• Vital signs observation, calculating QADDs score and taking appropriate action according to the QADDs score.
• Worked collaboratively as part of a multi-disciplinary health care team which included Doctors, Psychiatrist, Occupational therapists, physiotherapists, Social Worker, and Enrolled Nurses.
