  • Contracted 1 day per week to support the Grampians Region Palliative Care Consortium to develop a sustainable model that would build capacity of disability residential services staff in the Grampians region, in order to provide a Palliative Approach to residents in their care
  • Deliverables included: A Partnership Agreement (between Disability Residential Service Provider and Specialist Palliative Care Service), an End of Life Care Plan (for the person receiving care) and an End-of-Life Policy and Procedure Template (for organisations to implement in their service).
