While accepting the referral, team leaders can change the priority of assessment referrals. Undertake the triage process to confirm: • eligibility for an assessment • the assessment pathway • assessment priority – including whether the client urgently requires aged care services.
  • Undertake triage ensuring that clients are receiving a prompt assessment when required.
  • Assess to the latest standards and criteria.
  • Completed all IAT Training to undertake Simple and Complex assessments as part of the Single Assessment System.
  • Conduct comprehensive assessments of the care recipient’s care needs and help them access the most suitable types of care and services, including approval for government-funded aged care.
  • Undertake Support Plan Reviews for clients to determine if further assessment is required or to place more service referrals to assist in them at home and in the community.
  • Undertake Delegation to ensure that assessments are within all guidelines and are appropriate for the clients needs.
