Summary

I am an experienced and accomplished Medical Scientist with considerable years in a general clinical laboratory including Microbiology, Mycobacteriology, Haematology, Coagulation, Blood Bank, and Blood Transfusion. By virtue of having worked in different levels of the healthcare system from primary hospital laboratory with a lot of manual processes to Referral Laboratories I am quite experienced in many departments of the Pathology laboratory. Blood Transfusion was one department where I gained a lot training and experience in the bench as well as a Safety Officer. The experience of supervising other technicians and scientists in busy hospitals adequately prepared me for high standard customer care for both internal customers like clinicians, other departments and external ones like vendors, suppliers, engineers, primary health care facilities and other government departments. I am an individual with not only a passion of creativity, but I have a clear record of achievements in several areas where I worked as outlined below. The need of the skill of multi-tasking has been sharply on the rise in many organisations as it helps reduces “Muda” which means wastefulness of resources in the course carrying our business processes, and I am well suited to provide such as I am fully aware of the Lean principles.   • Company Overview: 137 bed government aided mission hospital in the South East District of Botswana. It is the largest hospital in the district and services a catchment population of 25,000 with a high number of drop-in clients from other villages and across the Botswana and South Africa due to its proximity to the Border.
  • Testing samples in haematology with Coagulation, clinical chemistry, blood transfusion, microbiology, flow cytometry, molecular studies and mycobacteriology.
  • Operation and maintenance of general laboratory equipment and major analysers
  • Ensure Laboratory’s consistent compliance to Ministry of Health Quality Standards covering risk management including patient safety, infection control and healthcare waste management.
  • Ensure Laboratory compliance to ISO 15189:2012 and guided it to transition from 2007 to 2012 and assessed in June 2018 and got accredited in December 2018
  • Investigated, re-evaluated and reviewed quality management system procedures as part of a planned Quality Management System program.
  • Reviewed patients report prior to release to the supervisor for eventual dispatch
  • Manage reagent and consumables through an excel based logistics management system and it helped maintained national expiry target within limits (

  • Achievements
  • Introduction of Viral Load Testing
  • The district did not have Viral Load Testing Machine but sent to a Referral Laboratory as per government system in place, but this led to high turn-around times, lost samples, re-bleeds and it affected patient care in that there were many delays in monitoring their ARV therapy.
  • This also led to patients being lost to follow as they were discouraged by lack of timely results and re-bleeds. I led the team of scientist in advocating for introduction of Viral Load Laboratory with the help of Ministry of Health Implementing partners; CDC Botswana.
  • An old building was renovated, new machines installed, and testing started in July 2017. This led to an improved TAT from 90days to within 14days with Priority Testing almost completely removed as TAT were very good. The project gave rise to improved patient outcomes as all clients received their results on time, shortening delays that were there before. The project was submitted to the Ministry of Health Annual Awards Committee
  • Advocated for Relocation of Molecular TB Diagnosis Testing
  • HIV and TB co-infection became a global challenge soon after TB war was won. Unfortunately, HIV entry into the scene dismally reversed all the good work done earlier. Soon, it became difficult to treat Tb and HIV as entities, so they were integrated. However, there was a challenge in that though HIV results could be known quicker, Tb results delayed due to the process of sample collection to testing to results dispatch.
  • Government introduced new method which could give a result not only of presence of Tb but also whether a patient has Rifampicin resistance or not. This knowledge by the clinician is vital in that the clinician can make a quicker decision on treatment regimen and hence help patient better with more information.
  • I advocated for the machine to be placed in the hospital where there were many samples and it was successfully done but not without huddles but my “It can be done” attitude as well team spirit it were valuable in the process. It was accepted and in August 2017 it was relocated and has since improved TB/HIV patient care as already detailed above.

  • Company Overview: LIMIH is a 380 bed Government operated hospital in the Ngamiland District of Botswana. It is the largest hospital in the district and services a catchment population of 55,000. LIMIH is unique in that it is in a remote part yet important tourism-rich part of the country. This provided unique challenges in providing a high level of service with suppliers of goods and services being far.

  • Provided training and monitoring of blood collection from all clinics with blood collection service to ensure good quality patients samples and hence quality results (Ensured that they followed good quality management guidelines on sample collection and at the same time maintaining smooth communication with all clinicians in the facilities on all issues affecting testing, sample collection and result dispatch.
  • Performing appraisals for all staff under my care in accordance to national performance-based reward system
  • Providing weekly reportable statistics to the district health team and Ministry of Health e.g Malaria

  • Achievements
  • Poor Customer Care
  • Many complaints were raised that bordered around poor customer relations. Laboratory staff was not responding to calls by the clinicians at night time and this delayed patient care and caused a plummeting of customer satisfaction results. Most of the calls were in relation to blood banking and transfusion.
  • I implemented quality improvement project. It involved instituting a 3 tier call for the reason of covering the blood transfusion calls as well as improving the communication and transport of blood from the national centre.
  • After getting approval we implemented a pilot for about 3 months and the complaints were drastically reduced. It was later agreed after all logistics of the call roster were finalised that it be adopted.
  • In 2009 the Laboratory was awarded a certificate by Ministry of Health as the Best District Laboratory in 2009 for achievements towards accreditation of pathology services

  • Company Overview: NBTS is wholly owned government department that provides collection and testing of safe and adequate donor blood for all health facilities in the country including the private sector. It is staffed by 10 scientists, 8 technicians and 3 Healthcare auxiliaries. The laboratory processed 28,000 blood samples /year.
  • Performed pre-analytical checks on received blood donor pints and pilot and troubleshooting for any errors
  • Processed blood namely Fresh Frozen Plasma, Platelets and Red cell concentrates within set standards.

  • Initiation of a formal Safety Department
  • At the formal introduction of Quality Management System in the department there was no single SOP for Safety and the Laboratory was easily accessed by anyone despite having sensitive client information like test results.
  • I volunteered to work in the safety without formal training and consequently managed to draft all Sops needed for a blood transfusion department. I would later receive formal training in safety, and I used the knowledge and skills learned to improve the drafts.
  • The SOPs were authorised and have up to date formed the backbone of the department’s safety section documentation. This achievement was acknowledged by my supervisor in my final performance review; I was said to be the most innovative officer in the department.

  • Company Overview: PMH is Botswana’s major tertiary and referral hospital, with a catchment population of 231 592 in Gaborone, plus patients referred from other areas of the country. The Hospital has about 530 beds and houses several specialty units. Its busy pathology department has Blood Bank, Biochemistry, Microbiology and Haematology with Coagulation Sections.
  • Registration of samples using manual and Integrated Patient Management System (Meditech) following sample collection guidelines provided like “order of draw”.
  • Participated in the 24/7 coverage of Biochemistry department covering all tests
  • Performed referral sample processing as per SOPs in the section e.g., special proteins, hormones and cancer markers (CEA), PSA
  • Performed equipment user maintenance which was daily, monthly and as needed liaising with engineers when and where necessary.
  • Participated in method validation process for new analysers placed in the section; analysers used were complex Coobas analysers with ISE modules as well as standalone ISE(Na+, K+, Cl- and Li+)

  • Introduction of Sample Collection Guidelines
  • The Laboratory received many samples which were collected on the wrong tubes because of lack of knowledge and this a great inconvenience to the patients as some clinics were quite far and most of the clients then were old people (Before HIV).
  • After my arrival at the facility as a new graduate I Initiated sample collection guidelines which though rudimentary provided visual assistance to the staff collecting blood in the clinics.
  • Soon after training of the clinicians on appropriate sample collection the rejection rate fell so drastically. Patient care was improved as instances of rejection dropped and doctors were better able to treat patients within short periods of delay.
  • Initiation of Microbiological Culture and Sensitivity Testing
  • When I arrived in the laboratory, I was surprised why such a busy laboratory could be sending samples away for culture. So, I enquired through the right channels and I was told it was because of lack of space and equipment.
  • My supervisor gave me go ahead to try and do something. I requested a small room which was a shower, renovated it a bit; requested equipment like autoclaves, incubators, anaerobic jars and initially ordered some media. The small microbiology laboratory started functioning after equipment and media were supplied.
  • Immediately after this I went for a brief attachment in the Referral Laboratory and we made an agreement that we will send plated that we could not identify due to lack of other things. Microbiology test reports TAT was improved and so did patient care. I have an attitude of “I can”, “We can” and with support of seniors all tasks and projects are always achievable.
