• Build relationships with patients, offering reassurance, active listening and engaging in therapeutic conversations
• Responded promptly to crisis situations, effectively managing severe mental health and behaviour issues
• Conducted independent nursing rounds to ensure proper care and document observations appropriately
• Facilitated therapeutic communication, conflict resolution and crisis intervention by redirecting negative behaviours and helping patients regain or improve coping abilities.
• Participated in the admission, discharge and transfer processes for patients
• Coordinate with the multidisciplinary team compromising of psychiatrists, social workers, nurses, occupational therapists, case managers and nurse unit managers.
• Supervised and mentored nursing students and junior nurses, providing guidance on workflow and patient care.
• Assume leadership responsibilities during assigned nurse in-charge shifts, overseeing nursing staff and coordinating patient care activities.

• Administer prescribed medications, monitoring patients for side effects and efficacy, and adjust interventions as necessary.
• Documented all patient interactions, treatments, and responses in electronic health records with accuracy and confidentiality.
• Collaborate with psychiatrists, psychologists, and social workers to develop and implement personalised care plans.
  

Second Rotation: [Mental Health Unit East - Inpatient Adult Acute Unit]
[Duration: 6 months]

• Provide crisis intervention services and support to patients experiencing acute mental health episodes.
• Enhanced my ability to manage crisis situations effectively, applying de-escalation techniques and supporting patients through acute mental health episodes.
• Educate patients and families on mental health conditions, coping strategies, and aftercare plans.
