Proactively ensure ongoing compliance, achievement of business targets,

Manage team workloads to ensure timely delivery of coordination requirements
to enable the effective execution of care plans. Resolve team resourcing issues
in a timely and prompt manner to ensure service delivery.
Involvement with recruitment as appropriate, including inducting, training and
mentoring team members.
Manage staff performance including regular supervision, annual appraisals and
performance management.
Foster a culture of transparent, effective, timely and appropriate communication.
Leads a team of Care Managers, ensuring that they provide exceptional customer service and are compliant with the Aged Care Quality Standards and relevant legislation.
Assisted with business development, strategy, and improvement of processes.
Provide clients with a hassle-free experience and wherever possible resolve issues/queries.
Monitoring annual reviews, income tested fees, monthly notes, debts, underspend, overspends, and compliance from the Care
managers in accordance with our internal company policies and the Aged Care Quality Standards.
ADHOC Case Management when required.
Manage the effective onboarding by the Care Manager of all clients within the agreed timeframes, including the care plan and budget.
Manage team workloads to ensure timely completion of care plans including the review and/or updating of individualized care plans to reflect changes in the client's needs.
Manage staff performance including regular supervision, annual appraisals, and performance management.
Assist the team with complex clients that have been assigned to them and coach and guide them as necessary.
Take on a client load assigned by the senior manager meet/make contact with clients and complete onboarding.
Coordinate services and appropriate care options to achieve positive outcomes for the clients and their families.
Provide a duty of care to clients by anticipating risks that could reasonably injure or harm them and attempting to mitigate the risk.
Provide emergency home visits when required.
Adhere to government regulations and guidelines.
Monitor changes in clients' needs and reassess as required.
Manage and monitor clients' budgets.
Maintain a current and accurate reporting system that meets auditing requirements.
