Setting - Designated ten bed inpatient Palliative Care ward.

• In collaboration with the multidisciplinary team implemented safe, compassionate, holistic care. With a focus on meeting the psychological, spiritual and physical needs of the patient with the ultimate aim of improving quality of life.

• Monitored, treated and provided care for ten patients each shift.

• Acted as a team leader/in charge most shifts. Supervising at least two staff.

• Acted as a Num several times to cover leave.

• Provided support, education and counselling to family members where appropriate.

• Conducted thorough patient assessments to identify changes in condition. Notified medical officer on the ward or on call and appropriate interventions implemented.

• Medications and procedures administered within my scope of practice and hospital policy adhered to.
• Assisted in the training and mentorship of new nursing staff members, contributing to a positive work environment and high-quality patient care.
• Skillfully managed challenging patient situations using de-escalation techniques, crisis intervention strategies, and therapeutic communication skills.
• Provided support for patients'' families during difficult medical decisions or end-of-life care, serving as a liaison between the healthcare team and loved ones.
• Advised patients and caregivers of proper wound management, discharge plan objectives, safe medication use, and disease management.
• Contributed to interdisciplinary team meetings by presenting relevant clinical data about patients'' progress toward reaching established goals of care.
• Demonstrated expertise in wound management techniques resulting in accelerated healing times for patients with complex wounds or pressure injuries.
• Documented treatments delivered, medications and IVs administered, discharge instructions, and follow-up care.
• Provided advice after hours to palliative pt's in the community. Calls ranged from simple medication queries to crisis situations requiring direct weekend admission.

• Supervising clients exploring alternative therapy's. This included monitoring vitals and urine analysis whilst client's were water/juice or broth fasting.
• Administering treatment's including but not limited to High colonic irrigation, Sitz baths-hydrotherapy, guided relaxation.
• Promoted a safe and comfortable healing environment by addressing patient concerns promptly and professionally.

Assisted residents of the home in all activities of daily living.
