I have worked in different specialization departments including General and medical ward, Intensive Care Unit in India and age care from last 4 yrs as Garde5 which give excellent consumer care and leadership skills.

• To ensure that staff work together collaboratively to provide quality nursing and care services to consumers.
• Carry out comprehensive and accurate nursing assessment of consumers. Assist in the formulation and modification of individualized care plans with the aim of ensuring that the optimal quality of life for the individual is maintained.
• Coordinate and monitor the implementation and evaluation of individual care plans to reflect any changes.
• Participate actively in Cumberland Manor's ACFI process and alert Centre Manager to changes in resident's health status.
• Liaise with other health professionals to discuss consumer care needs. Maintain accurate and legally appropriate documentation of care provided within the shift.
• Communicate with cohort and other team, external and internal providers which include medical officers, and other health care providers as well as families and consumers.
• Practices in accordance with WHS and safe work procedures in the facility and ensures that all incidents are correctly documented, acted upon and reported according to policy and procedures.
• Monitored elder's condition and assessed their needs to provide the best possible care and advice.
• Supervised the other staff especially in COVID outbreak to prevent the further infection.
• Interpreted and adhered to established nursing and aged care standards and practices. Built and maintained positive relationships with medical personnel, elders, and their families.
• Collaborated with a team to plan for elder's care during this pandemic outbreak.
