Tasks and duties :

• Conduct Handover
• Read Dairy/ Communication book and memo file
• Discuss any changes in health statues with RN/ CN and Supervisor Carer
• Ensure staff are appropriately allocated
• Check in on unwell patients
• Liaise A.C.F.I coordinator and check all assessments
• Review accident/incident forms completed
• Review Wound Care
• Check and discuss with Facility Manager any changes in health statues of residents and any additional issues
• Review PRN forms and Medications
• Care Plan reviews
• Complete new admissions and any new assessments eg. falls/pain
• Liaise with GP and other health professionals
• Supervise Lunch
• Assess residents and ensure the correct assistance for dietary preference and nutritional intake
• Check stock supplies
• Order any stock needed
• Conduct or Attendant Family Conferences
• Report any issues to the Facility Manager
• Complete staff Appraisals

Tasks and duties :

• Assisting patients with their personal care needs such as showering, toileting, shaving, oral hygiene, dressing and eating.
• Assisting patients with their mobility and communication needs.
• Following therapy plans such as interventions to assist those with dementia and behavioural problems.
• Observing and reporting changes in patients' condition.
• Assisting with rehabilitation exercises, basic treatment and delivering medications.
• Assisting nursing and midwifery staff with routine technical support functions including setting up for procedures, cleaning equipment and completing basic documentation.
• Maintaining accurate and detailed patient records.
• Building effective relationships with patients through clear communication and empathy.
• Applying simple wound dressings.
