Family and Child Connect service

Family and Child Connect is a local Community Based Intake and Referral Service connecting families to the right support at the right time. Worker role:

• Provide advice, information, assessment and referrals to vulnerable families
• Work with primary care givers from a whole of family perspective to achieve improved outcomes for the children
• Support families with complex needs, including but not limited to, domestic and family violence, homelessness, disability, mental illness, trauma, drug and alcohol misuse, criminal activity, financial hardship, family law and social isolation
• Build professional relationships and knowledge of a broad range of services available in and around our region
• Provide advice and information to professionals to support the direct linking with supports for their clients

• Providing personal care to clients using prescribed equipment.
• Liaising with other health care professionals such as physiotherapists
• Assisting residents with activities of daily living such as bathing, feeding and dressing.
• Observing and reporting quickly any changes in clients’ well-being or behaviour.
• Assisting service users with their environment & day to day activities.
• Making recommendations for changes and improvements to client’s care.
• Organizing and assisting with recreational activities.
• Assist clients with their exercise, physiotherapy and other medical plans.
• Maintaining confidential client files and information as required.
• Assisting with prescribed medication.

• Part of Young People’s Advisory Committee which organised different community projects and participated in decision making at the center
• Participated in CAMP (LGBTQI+ peer support group)
• Co-facilitated a psychosocial skills program on anxiety at Alexandra Hills TAFE
• Successfully created engagement plan, developed and designed community awareness project which involved creating relationship with another organisation
• Researching community organisations and updating databases
• Conducted psychosocial assessments

• Organizing and facilitating play for the children
• Engaging with parents and caregivers and assisting with references
• Preparation of meals

Sing And Grow program

• Organized and facilitated play for the children
• Participated in the musical activities with the parents

Asylum Seeker and Refugee Assistance Program (ASRA)

• Administrative duties such as organizing appointments
• Assisted with employment, i.e., preparing resumes and submitting applications for clients
• Assistance with paperwork
• Assistance with referrals and access for emergency services
• Assessments for access eligibility
• Translator assistance- assisted non-English speakers with anything that needed Translating and Interpreting services (TIS Australia)

Woodridge State school

• Breakfast club- preparing breakfast for the students every morning
• Homework club- helping mostly immigrant children with school assessments
• English classes- assisting English teacher for refugees and asylum seekers
• Playgroups- organized for children under 6 mostly refugees and asylum seekers
• Kindy applications- filled in and applications for grants for children through Access
• Foodbank- distributed food to refugees and asylum seekers in Logan area
• Translator assistance- assisted non-English speakers with anything that needed Translating and Interpreting services (TIS Australia)
• Assistance with paperwork, e.g., Centrelink documents
