Senior Institutional Social Worker/ AHP2

  • Completing a range of risk assessments including those with a suicide (SAMI) and crisis intervention focus
  • Facilitating Making Changes Program (MCP) which is a moderate intensity program designed to address Alcohol and Other Drug use and a general risk of re-offending, with up to 12 male prisoners
  • Working closely with the Cultural Development Coordinator (CDC) and Aboriginal Liaison Officers (ALO) to provide culturally sensitive intervention to First Nations People
  • Worked collaboratively with NDIS participants (new and existing) and key stakeholders through the planning, implementation and review of their NDIS plans
  • Advocated for NDIS participants to receive services to achieve their goals, cease reoffending and successfully transition back into the community
  • Worked collaboratively with Aged Care consumers to apply, be assessed for and access aged care services (ACAT)
  • Complete extensive throughcare planning (inclusive of housing, mental health, personal and intervention needs) to support successful transition and reintegration into the community after many years in custody
  • Working collaboratively with SA Prison Health Service and a range of allied health professionals to meet prisoner health care in relation to prisoners with complex psychological and mental health diagnosis
  • Assigned Social Worker for Aged and Infirmed Unit (elderly and disabled), Greenbush Unit (complex behaviours and protectee prisoners) and until recently the Aboriginal Accommodation Unit (housing traditional First Nations people)
