Megan Denby

I have worked in a variety of roles at Bass Coast Health ranging from case managing in the community as a Transition Care Coordinator to working on the acute and sub-acute wards as well as the residential aged care facilities as either an RN on the floor or an Associate Nurse Unit Manager. I am currently working as a Clinical Development Nurse working in our education department supporting and educating students and nurses.

- Facilitated all new admissions into the residential aged care facility

- Supported prospective residents and their families with the process of entering residential aged care

- Liaised with health care services in the community and assisted with referrals

- Met financial and occupancy targets set by Blue Cross

- Case managed clients living in their own homes on the Transition Care Program and Hospital Admission Risk Program

- 'Acting' Health Independence Program Manager during the month of November 2014

- Worked on the road visiting clients in their own homes providing nursing care.

- Worked in the customer service centre giving over the phone support and advice to other nurses on the road and clients

- Undertook the role of care manager, orientating new staff, supporting RNs with complex and difficult clients and performing the role of 'centre contact'

- Graduate preceptor for the year of 2011, supporting and educating graduate RNs

- Completed 'graduate year' and gained experience working on orthopaedic, general surgical and medical wards
