taking workload of four patients and providing holistic nursing care for patients with different renal problems under the supervision of the RNs.
320 hours general medicine ward placement experience.
Clinical assessment and observations: Procificent in vital signs observation, Fluid balance chart, blood glucose monitoring, neurovascular assessment, neurovascular assessment, ECG monitoring, mental health status, INR assessment, U/A analysis, etc
And enable to document correctly in the EMR system and interpret the trend or the signs of deterioration
Medication management: Administer oral/ subcut/ IV / IM medication safely following in the protocol Confident with using the IV pump, Nikki pump
Using the Australian injectable medication book as evidence-based practice
infection control: Consistent application of infection control practices, including hand hygiene, PPE usage, and maintaining aseptic technique
Skin care and wound care: Great understanding of pressure injury prevention and management,
Confident with using aseptic technique for wound care, observed the wound status and documented the wound care Able to identify the sign and symptoms for infections, able to collect the wound swab
Time management: Ability to manage and prioritize multiple tasks, from medication rounds to charting, without compromising care quality
Communication with patients and families: Clear, compassionate communication with patients and families, especially when delivering complex or difficult information
Able to provide patients and their families clear and brief education about medication administration, care plans with a focus on health literacy
Collaboration and teamwork: Working effectively with physicians, allied health professionals, and other nurses to coordinate care and develop treatment plans
