• In my first role upon completion of internship, I have assumed a much greater degree of responsibility in terms of decision making and patient care
• As Senior House Officer in the ambulatory area of the acute medical assessment unit, my primary role involves the review and care of patients who either presented via emergency department triage or directly by GP referral
• On the basis of the history elicited, examination findings and clinical judgement I am responsible for requesting investigations in the acute setting, involving appropriate in-house medical + surgical specialties and discussing differential diagnoses with my consultant in order to form a suitable management plan
• I have also had the opportunity to practice ward based medicine as I have assumed care of patients admitted under the AMAU, and have furthered my clinical skills to include tasks such as lumbar puncture

• In my final rotation of my internship, I held the majority of responsibility for management of inpatients both pre and post operatively
• This role requires a considerable degree of multidisciplinary cooperation with physiotherapy, occupational therapy, ortho geriatrics and other medical and surgical specialties
• As a ward based intern I acted as the primary point of communication between my team and these disciplines
• I was also responsible for pre-operative assessments for elective surgeries in Merlin Park University Hospital as well as safe and medically responsible discharges with appropriate follow up

• Worked as part of a large team in both ward based general medicine and rheumatology outpatient clinics
• Our inpatients consisted of a vast array of medical presentations on account of our team participation in general medical call

• I assisted in the management of a wide variety of presentations including respiratory, cardiac endocrine and psychiatric pathologies
• I also independently reviewed out patients in general respiratory and sleep apnoea clinics and upon presenting history and examination findings to my consultant, assist in the formulation of management plans
• I had extensive opportunity to exercise my clinical judgement and procedural skills as a ward based intern on call in cooperation with a medical registrar on call, covering 11 wards and approximately 150 inpatients at a time
• I also participated in teaching final year medical students through ward based short case preparation and practice

• I received my first exposure to hospital medicine in this role
• I worked on a small general surgical team consisting of myself, an SHO, a registrar and my consultant
• I was largely responsible for managing acutely deteriorating inpatients on the wards and formulating management plans in these instances
• I also had the opportunity to assist in endoscopy theatre with patient consenting and cannulation
• Organisational skills and time management were critical in this role with a high turnover rate of patients
• I was also responsible for safe discharge with appropriate follow up
