- Provide high quality direct care support and supervision to older, and young people
- Provide nutritious well-balance meals encouraging healthy eating patterns in line with agreed dietary goals and team agreements
- Ensure all documentation is up to date and ready for handover at the end of each shift
- This including but is not limited to data entry, client’s observation sheets, case notes diaries, meetings records, report templates and incident reports
- Experience in providing flexible solution focused case work and support to older/ young people
- Ability to effectively engage with older/ young people who may be demonstrating challenging behaviors and / trauma related behaviors associated with child neglect/ abuse
- Provide individualized support to clients and assisting them in improving quality life
- Assisting clients in day-to-day activities from personal hygiene to taking meals and drugs
- Using wheelchair and stretchers and transporting patients for appointment and shopping
- Provide personal assistance and care to indoor and outdoor clients
- Provide clients to access activates and outings in community
- Provide direct care assistance to each client in accordance with their service plan and as directed by manager
- Accompany aged and disables persons during daily activities
- Assisted clients on matters of personal care including bathing, dressing, eating and medication
- Assists clients with their mobility
- Ensure the safe and proper lifting and transferring of clients
- Assisted individuals to perform a range of housekeeping functions and develop their level of ability in all aspect of daily life
- Providing individuals support to patients and assisting them in improving quality of life
- Lending compassionate support in fulfilling complex needs of patients
- Perform light housekeeping duties such as dusting, washing dishes, folding laundry
- Ensure that client’s food is prepared according to a specific healthcare plan and assist them in partaking their meals
- Provide them with companionship
- Monitor changes in client’s behaviors and report any significant observations to families and health care provider
- Ensure clients dignity by keeping personal and medical information confidential
- Ensure a clean, safe and healthy environment for patients to help them thrive both physically and emotionally
- Assist clients with both active and passive range of motions as per the care plan
- Observe and document changes in client’s behaviors.
