Population Health Planning Program (Bentley-Armadale Medicare Local). Strategic management role overseeing BAML Population Health Planning and Research. The role reported to the Chief Executive Officer. The role was primarily responsible for developing BAML's needs-based planning for managing comprehensive primary health care and population health across the Bentley-Armadale Medicare local region. Currently managed roles/programs include the following.
Population Health Planning Program:
Provide comprehensive and timely advice to the CEO and BAML Executive Management team on emerging and existing primary health care and population health issues and capacity-building opportunities through:
- Skillfully utilise contemporary planning tools, methodologies (including epidemiological analysis) and frameworks to scope, develop, implement, evaluate, monitor and report on BAML's strategic responses to primary health care and population health issues.
- Apply principles of evidence-based practice; implemented structured approach to strategy development and evaluation, needs identification, priority setting, risk management, project, change management and quality improvement.
- Lead a team 15 both medical and allied health clinicians and health administrators that delivered exceptional, wide-ranging community health need assessments.
- Identify funding opportunities, prepare funding tenders, submissions and proposals and make recommendations for endorsement by the Medicare Local.
- Facilitate and prepare reports, briefing notes and responses for key stakeholders including the BAML Executive. Provide input into the strategic direction of Medicare Local.
Stakeholder relationships
- Liaise with external bodies and institutions to ensure up-to-date knowledge and understanding of Government and relevant non-government organisation policy and programs.
- Facilitate an area wide approach to health needs assessment, primary care service integration and the development of sustainable solutions through strategic research, planning, evaluation and partnership management.
Project management
- Manage service development initiatives including the dissemination of information and development of service models, frameworks and resources for the successful translation of new knowledge into practice. Develop action plans and business proposals and manage project to closure and evaluation.
Procurement & Clinical Service Design
- Provided funding support for the provision of comprehensive face-to-face after hours services in their BAML.
- Establishment of community based Iron infusion therapy clinic, Belvidere St Health Centre, Belmont - 2013.
- Iron infusion therapy Service in RACFs: an outreach services, 2015.
- Establishment of COPD clinic, 2014.
- Aboriginal Health Care Coordination in Medical Hostels Trial 2014-15.
Afterhours primary care program.
- Provided funding support for the provision of comprehensive face-to-face after hours services in their BAML region. Project managing funding for this is sourced from redirected Practice Incentive Program (After Hours Incentive) payments, the GP After Hours grants scheme and additional targeted new grants. Under this program, Medicare Locals are responsible for the administration of dedicated funding to further improve access to afterhours care. This includes working in
consultation with local communities to develop and implement regionally tailored after hours service models, building on existing services where possible.
Outer Metropolitan Relocation Incentive Grant (OMRIG).
The Programme was designed to meet the Government’s commitment to outer metropolitan areas. It encouraged eligible general practitioners and specialists, who currently work in inner metropolitan Perth areas to relocate to, and remain in, outer metropolitan regions. This included:
- Medical practitioners who are currently working part-time or who are temporarily out of the workforce.
- Administer 2013-14 OMRIG funding agreements between the Medicare Local and medical practitioners in accordance with the Programme Guidelines.
- Promote OMRIG within the Medicare Local’s boundary (Southern metro Perth).
- Undertake verification of medical practitioner details.
- Participate in meetings/teleconferences to be held as required, between the Department of Health and other relevant stakeholders.
- Provide a letter of support to applicants seeking to relocate under the Programme. Letters of support must address the net benefit to the community.
- Collect and provide information/data on the Programme for reporting purposes as requested by the Department of Health.
- Manage enquiries and correspondence from medical practitioners relating to eligibility.
- Regularly consult with the Federal Department of Health in relations to policy and operational matters associated with OMRIG.
General Practice/Primary Care Sector Support
The BAML General Practice Support program incorporates many services to provide accessible and up-to-date information and resources and guides to support General
Practices within the catchment. The areas of focus for the General Practice Support program included the provision of
resources for practice accreditation and re-accreditation; Medicare Australia updates; IPs and SIPs; chronic disease management; medical software templates, data cleansing,
practice and human resource management. The Practice support team also distributed WA Health communicable disease alerts encourages practices to remind and recall and provide information on the availability of training and education opportunities for all
practice staff.
National eHealth Change and Adoption Initiative
Support and implement the eHealth initiatives in the Bentley-Armadale Medicare Local region including (but not limited to):
- rollout of the Personally Controlled Electronic Health Record (PCEHR).
- Identify the level of eHealth awareness and readiness of the local community (consumers and providers) and assist in developing a local Change and Adoption Plan to meet identified needs.
- Drive awareness of the eHealth record system amongst primary health care providers and consumers locally.
- Provide training and support for clinicians and practices in activities involved in eHealth readiness and adoption.
- Increase information technology capacity and eHealth readiness in the General Practice setting.
- Support local general practice to meet eligibility requirements for the eHealth Practice Incentive Program (ePIP).
- Support patient consent and registration processes for the eHealth record system.
- Facilitate coordination and integration across providers using the eHealth records system and eHealth more broadly including LHNs, and allied health.
- Report on the eHealth experience and achievements by Medicare Local including participating in surveys and reporting activities managed by the AML Alliance to understand achievement, progress and barriers to adoption of eHealth in primary health care in the Region.
- Supporting primary care providers in improving data quality; and maintenance of the National Health Services Directory (NHSD).
