Summary

When I commenced my role as a Care Coordinator I felt that I had hit the jackpot! I am able to utilize all my years of experiences across different settings and my life experiences, in order to provide optimal care to patient as well as directly influence flow through the Emergency Department. My job is extremely challenging and rewarding, and I am ready to now further expand on it.

I am a highly enthusiastic and responsible clinician, working both independently and as part of a wider team. I love to share my experiences with colleagues and in turn also learn from them.

I am passionate about what I do, and believe that the trans-disciplinary model of allied health care is of huge benefit to patients, as well as enables efficient service. I do my best work when I am involved in direct patient care, but am also aware of the importance in being involved in other projects that will enhance clinical skills for myself and the wider Health Care Network.

Work History

Care Coordinator - Emergency Department

Monash Health

Trans-disciplinary role incorporating Physiotherapy, Occupational Therapy and Social Work elements in order to provide wholistic input and individualized interventions.

  • Independently mange referrals, ensuring timely care to coordinate effective discharge planning and facilitate patient flow through the Emergency Department, as well as aim to avoid representation to the Emergency Department.
  • Streamlining the coordination of services for patient, including early referrals to relevant other Allied Health Disciplines.Developed strong relationships with team members within the Emergency Department as well as other Health Services.
  • Communicate clearly to the treating team the outcome of any input, both verbally and in files, in a timely manner.
  • Maintained accurate and up-to-date documentation of patient records in accordance with employer regulations.
  • Support less experienced members of the Care Coordinator team.

Scope of practice includes, but is not limited to:

  • Assessment of mobility/transfer status, social situation and supports, functional capacity, home setup and cognition.
  • Provision of aids/equipment as required to facilitate discharge.
  • Educate patients on strategies to promote independence and empowering them to manage their health conditions.
  • Ensuring appropriate follow up in the community is arranged and patient/caregiver is aware of how to access these services.
  • Screening and support for Family Violence, including Elder Abuse.
  • Quickly build trusting relationships with clients/caregivers in order to provide input within a short time frame, including offering emotional support when needed, such as in situations of grief and adjustment to injury, unexpected hospitalization or separation.
  • Advocate for patients' rights within the healthcare system, working to address concerns or barriers to discharge or care.
