Trans-disciplinary role incorporating Physiotherapy, Occupational Therapy and Social Work elements in order to provide wholistic input and individualized interventions.

• Independently mange referrals, ensuring timely care to coordinate effective discharge planning and facilitate patient flow through the Emergency Department, as well as aim to avoid representation to the Emergency Department.
• Streamlining the coordination of services for patient, including early referrals to relevant other Allied Health Disciplines.Developed strong relationships with team members within the Emergency Department as well as other Health Services.
• Communicate clearly to the treating team the outcome of any input, both verbally and in files, in a timely manner.
• Maintained accurate and up-to-date documentation of patient records in accordance with employer regulations.
• Support less experienced members of the Care Coordinator team.

Scope of practice includes, but is not limited to:

• Assessment of mobility/transfer status, social situation and supports, functional capacity, home setup and cognition.
• Provision of aids/equipment as required to facilitate discharge.
• Educate patients on strategies to promote independence and empowering them to manage their health conditions.
• Ensuring appropriate follow up in the community is arranged and patient/caregiver is aware of how to access these services.
• Screening and support for Family Violence, including Elder Abuse.
• Quickly build trusting relationships with clients/caregivers in order to provide input within a short time frame, including offering emotional support when needed, such as in situations of grief and adjustment to injury, unexpected hospitalization or separation.
• Advocate for patients' rights within the healthcare system, working to address concerns or barriers to discharge or care.

Home based rehabilitation service:

• Provide interventions and treatments to improve patient abilities to perform daily activities, manage symptoms and prevent injury.
• Emphasis on optimizing safety at home, often with the use of equipment/aids and home modifications.
  Ensuring appropriate engagement with community resources and supports.
• Enhanced overall quality of life for patients with chronic conditions by addressing physical, cognitive, psychosocial, and sensory impairments.
• Established compassionate and caring relationships with patients and caregivers, including monitoring for carer stress.
• Apply for funding through Home Care Packages and NDIS, using clinical justification for requests.

Casual employee across different sites and wards, including Acute, Inpatient Rehabilitation, Outpatient Rehabilitation and Palliative Care:

• Focus on discharge planning and improving patients' abilities to perform daily activities and manage symptoms.
• Ward based care including room setup and equipment for use in hospital and pressure care.
• Arrange equipment and services for end of life care at home, often with in very short time frames.
• Individual and group sessions, including presentation to family/carers.
• Collaborated with multidisciplinary team members for comprehensive patient care and optimal results.

Volunteer Role:

• Managed all aspects of uniform supply to over 1200 students, including ordering, finances, stock control, product selection and sales.
• Trained up team of over 30 volunteers.
• Arranged for complete overhaul of processes, including stock control and sales.
• Developed and maintained relationships with customers and suppliers.
• Reduced operational costs through improvement initiatives and resource management, to ensure business could operate as not-for-profit and give back to those in need in the school community.
• Ongoing communication with senior stakeholders regarding policies and implementing a new uniform in 2025.
• Always responding in a timely manner to all queries from parents, students and staff.
• Maintain a professional, organized, and safe environment for volunteers and families.
