• Complete accurate and comprehensive documentation and communication in the client information management system and with referrers, service providers and stakeholders in accordance with Merri Health documentation policy and best practice
• Work closely alongside the Occupational Therapist (NDIS assessor)to ensure the client journey is streamlined
• Liaise with multidisciplinary team members, support coordinators, external organisations and other referrers such as GPs /funding bodies
as required
• Occasional home and community visits to support clients in accessing resources and evidence for the NDIS application
• Respond to client queries in an appropriate and timely manner and discuss with the line manager as required.
• Work with the client to identify other supports if needed during assessment that is not funded by the NDIA or if immediate escalation of care is required
• Ensure that services are accessible, culturally relevant and inclusive and service delivery reflects the cultural and social diversity of the local
community
